Summary
Investigations of human Paneth cells with regard to orthology and pathology were performed on more than 2725 biopsy-, operation- and autopsy-specimens. As a rule the Paneth cells are found only in the small intestine and appendix, their number being greatest in the terminal ileum. The presence of singly scattered Paneth cells along the fundo-antral borderline and in the caecum is regarded as heteroplasia without any functional-pathogenic significance.
In different gastroenteropathies oxyphil granular cells develop at sites where they do not occur under physiologic conditions. This is especially true in chronic atrophic gastritis with intestinal metaplasia, in ulcerative and neoplastic gastric diseases, as well as in infectiousulcerative and neoplastic diseases of the rectum and colon. The oxyphil granular cells however are not regarded as integrale parts of a tumour or tumorous conditions. The occurrence of Paneth cells in the ileum in Crohn's disease apparently depends on the grade of the disease.
From the ulcerative colitis three different cell-types can be distinguished all having cytoplasmic granules. Two of these cell-types (type I and II) are at least partly identical with typical Paneth cells of the small intestine. The third type is concerned with immunoglobulin secretion.
In formal-genetic aspect the development of oxyphil granular cells in the stomach and in the colon is regarded as indirect metaplasia. The function of Paneth cells as well as of oxyphil granular cells is still a point of discussion. The Paneth cells of the small intestine and of the appendix seem to have an antibacterial role by continously secreting lysozymes. At least a part of the metaplastic oxyphil granular cells seem to have a similar function.
Zusammenfassung
Untersuchungen zur Orthologie und Pathologie menschlicher Paneth-Zellen wurden an über 2725 Biopsie-, Operations- und Autopsiepräparaten durchgeführt. In der Regel treten Paneth-Zellen nur im Dünndarm und in der Appendix auf; ihre Entwicklung ist im (terminalen) Ileum am stärksten. Singuläre Paneth-Zellen im Bereich der fundo-antralen Grenzzone und in Coecum werden als Heteroplasien ohne funktionell-pathogene Prospektivität gewertet.
Verschiedene Gastroenteropathien führen zur Entwicklung oxyphiler Körnerzellen auch an Orten, wo sie unter physiologischen Verhältnissen nicht vorkommen. Insbesondere handelt es sich um chronisch-atrophische Gastritiden mit intestinaler Metaplasie, um ulceröse und neoplastische Magenprozesse sowie um entzündlich-ulceröse und neoplastische Veränderungen des Colon und Rectum. Innerhalb der blastomatösen Prozesse allerdings konnten oxyphile Körnerzellen als integrierte Tumorkomponente nicht bestätigt werden. Das Verhalten Panethscher Zellen im Ileum bei Morbus Crohn ist offensichtlich abhängig vom Entwicklungsstadium der Erkrankung.
Bei Colitis ulcerosa lassen sich 3 verschiedene Zelltypen mit granulären Cytoplasmaeinschlüssen differenzieren. Zwei dieser Zellformen (Typ I und II) sind zum Teil wenigstens mit typischen Paneth-Zellen des Dünndarms identisch; bei der dritten handelt es sich vermutlich um Immunglobulin-sezernierende Zellen.
Formalgenetisch wird die Entwicklung oxyphiler Körnerzellen im Magen und Colon als indirekte Metaplasie aufgepaßt. Die funktionelle Bedeutung sowohl der Paneth-Zellen als auch der metaplasiogenen oxyphilen Körnerzellen ist noch immer umstritten. Für die Paneth-Zellen des Dünndarms und der Appendix wird eine durch kontinuierliche Lysozymsekretion unterhaltene antibakterielle Rolle diskutiert. Eine entsprechende Funktion wird zumindest auch für einen Teil der metaplasiogenen oxyphilen Körnerzellen angenommen.
