Zusammenfassung
Bei drei Hypernephrompatienten tritt 12 Std nach Lymphographie eine über Wochen röntgenologisch sichtbare Kontrastmittelansammlung in den Lungenmetastasen auf. In einem Falle werden 4 Wochen nach einer zweiten Lymphographie mit radioaktiv markiertem Jod131-Lipiodol-UF (Guerbet) histochemische (Halogenadditionsmethode) und autoradiographische Untersuchungen durchgeführt, die eine Speicherung des Jod-Lipidkomplexes in den Hypernephromzellen der Lungenmetastasen bestätigen. Mehrere Faktoren, vor allem aber die schlecht voraussehbare Kontrastmittelmenge, die nach der Lymphographie in die Lungenstrombahn gelangt, bestimmen die unterschiedliche Kontrastmittelkonzentration in den Lungenmetastasen bei den einzelnen Patienten. Ob sich aus dem Lipidspeicherungsvermögen dieser Tumoren einmal diagnostische und therapeutische Konsequenzen ergeben werden, ist noch nicht abzusehen.
Summary
In three patients with hypernephroma the contrast medium, Lipiodol-UF, Guerbet, accumulated in pulmonary metastases 12 hours after lymphography. It was visible for several weeks with little change in its radiographic appearance. The radiopaque material was observed as scattered, fine droplets throughout the metastases, frequently in a ring shape. Four weeks after a second lymphography in one patient, using I131-tagged Lipiodol-UF, histochemical (halogenation method) and radioautographic investigations were made which disclosed an intracellular amassing of the iodinated lipid complex in the pulmonary metastases. Several factors determined the concentration of contrast material in the metastases of the patients. First, the poorly estimated quantity of oily media which entered the venous circulation and pulmonary vascular bed after lymphography; second, the variations in the vascularization, and the lipid composition and concentration of the metastases and their relative surfaces. Quantitatively determined amounts of Lipiodol-UF will be injected intravenously, possibly with emulsion of iodinated lipids. At present it is impossible to say whether the capacity of these tumors to store lipid will have diagnostic and therapeutic value.
