Zusammenfassung
An Hand von 29 kindlichen Erkrankungsfällen der Jahre 1960–1963 wird über hirnelektrische Befunde im akuten Stadium der Poliomyelitis acuta anterior, in der Rekonvaleszenz und später berichtet.
Zunächst ist das EEG in über 50% abnorm, wobei die Befunde variieren von einer Verlangsamung der Grundaktivität über diffuse und umschriebene Dysrhythmien bis zu krampspezifischen Störungen. Die hirnelektrischen Veränderungen zeigen eine Beziehung zur überwiegenden klinischen Ausprägung der Erkrankung (Spinalparalyse, Bulbärparalyse, Encephalitis, Meningitis). Überdies scheint der Erregertyp auf Form und Schwere der Erkrankung und damit die im EEG sich spiegelnde cerebrale Funktionsstörung Einfluß auszuüben. Je früher im Verlauf der Erkrankung das Hirnstrombild abgeleitet wird, um so häufiger finden sich Veränderungen. Diese lassen sich bei der Mehrzahl bis 6 Monate nach Erkrankungsbeginn nachweisen.
Nach Ablauf eines Jahres hat sich das EEG der kritisch ausgewählten Fälle normalisiert.
Spätfolgen werden dann beobachtet, wenn der Poliomyelitis vorausgegangene oder mit ihr gleichzeitig wirksam werdende Noxen zur Entstehung eines schwereren Hirnschadens beitragen.
Die Poliomyelitis als Ursache bzw. Teilursache eine Residualepilepsie wird diskutiert und als möglich erachtet.
Summary
It is referred to electroencephalographic findings during the acute phase, the convalescence and later of anterior Poliomyelitis seen in 29 children 1960–1963.
Primarily the EEG is pathologically altered in more than half of the cases, findings ranging from moderate nonspecific abnormality with reduced frequency of the background activity, diffuse and localized dysrhythmia to seizure activity. The electroencephalographically registered changes are dependant upon the preponderant clinical expression of the disease (spinal paralysis, bulbar paralysis, encephalitis, meningitis). Furthermore the type of Poliovirus appears to influence the extent of the disease and therewith the degree of disturbed cerebral function reflected by the EEG. The earlier in the course a tracing is obtained the more frequently are abnormalities seen. These persist in the majority as long as 6 months after onset.
The EEG of the critically selected cases had returned to normal within one year.
Longstanding changes could be observed where insults prior to the Polioencephalitis or concomitating this disease probably had prepared more serious cerebral damage.
Poliomyelitis as a cause or partial cause of residual epilepsy is discussed and regarded as a possibility.
