Das klinische Bild der akuten Salmonellengastroenteritis beim Kind

Zusammenfassung

  1. 1.

    An einem Krankengut von 130 Kindern, die frei von anderen Begleitkrankheiten waren, mit bakteriologisch nachgewiesener akuter Salmonellose (Enteritisgruppe) wird mit klinisch-statistischen Methoden das Krankheitsbild der Salmonellen-Gastroenteritis untersucht.

    Bei Säuglingen dominiert das Leitsyndrom: Fieber, Durchfall und Erbrechen. Als Nebensymptome fanden wir Lebervergrößerung und Exsiccose, während Roseolen und Milzvergrößerung fehlten. Der Nachweis von Salmonellen im Urin, die Leukocyturie, geringe Albuminurie und Bacteriurie deuten auf eine Beteiligung des Urogenitaltraktes hin.

    Bei älteren Kindern sind Fieber und Erbrechen stärker ausgeprägt. Der durchfällige Stuhl ist weniger frequent und zeigt seltener Blutbeimengungen. Als Nebensymptome wurden Somnolenz, brauner Zungenbelag, Splenomegalie, Roseolen, Exsiccose und Kollaps gefunden. Rachenrötung und bronchitische Rasselgeräusche werden mit steigendem Alter häufiger beobachtet.

    Unsere Ergebnisse werden an Hand der vorliegenden Literatur eingehend diskutiert.

  1. 1.

    The pathological picture of acute salmonella-enteritis (enteriticgroup) were clinically and statistically seen in 130 children, who were bacteriologically diagnosed, but free from other diseases.

  2. 2.

    The prominent symptoms in infants are fever, diarrhea and vomiting. The uncommon symptoms are hepatomegaly and dehydration. Rose spots and splenomegaly are absent. The presence of Salmonella, leucocytes and a few albumin in urine will interpret the urinary tract infection, which is just a part of salmonellosis in infants.

    In older children fever and vomiting are more frequent. Diarrhea is less common and blood in stool was rarely found. The seldom symptoms are drowsiness, a brownish furred tongue, splenomegaly, rose spots, dehydration and collaps. The older the child is, the greater is the frequency of inflammation of the throat and rhonchi on auscultation of chest.

    Our results are discussed on the basis of aviable literature.
