The entry and course of the oculomotor, trochlear and abducent nerves in the cavernous sinus have been studied by preparation, histology and by means of injection and resin corrosion casts.

The oculomotor nerve enters the transversal plate of the cavernous sinus either rostral, lateral or occipital of the posterior clinoid process. At first it traverses a recess of the dura mater which is 8 mm long on the average and has a dead end under the anterior clinoid process. A narrow arachnoid pouch (=cisterna n. oculomotorii) follows the nerve up to the dead end of the recess.

The point of entry of the trochlear nerve varies in position. In about 75 per cent of cases the nerve enters the cavernous sinus from above and in about 25 per cent from the lower side of the tentorial incisure. Along its “intradural” course in the upper and lateral wall of the cavernous sinus and the plica petroclinoidea, respectively, the nerve is enclosed by an arachnoid sheath.

The abducent nerve penetrates the dura mater by an oval gate in the clivus region. The point of entry varies in distance from the posterior clinoid process. The abducent nerve is also enclosed for a short distance by an arachnoid sheath.

Many variations with regard to points of entry as well as to the intradural course of the oculomotor, trochlear, and abducent nerves have been found.

The shaping of the plica petroclinoidea and the mouldings formed by the nerves show many individual variations.

Eintrittsorte und Verlaufsstrecken in den “Sinus cavernosus” des III., IV. und VI. Hirnnervs werden präparatorisch, histologisch und an Injektions-und Korrosionspräparaten untersucht.

Der III. Hirnnerv tritt entweder vor, neben oder hinter dem Processus clinoideus posterior in die Transversalplatte des Sinus cavernosus ein. Hier zieht er zunächst in einer durchschnittlich 8 mm langen Duratasche, die unter dem Processus clinoideus anterior blind endet. Eine enge Arachnoidaltasche (=Cisterna n. oculomotorii) begleitet den Nerv bis zum Taschenende.

Die Eintrittsstelle des N. trochlearis ist unterschiedlich gelegen. In ca. 75% tritt er in den hinteren Abschnitt der Wannenregion von oben her ein. In ca. 25% zieht er in die Unterseiter der Incisura tentorii selbst ein. In seinem “intraduralen” Verlauf in der Ober- und Seitenwand des Sinus cavernosus cavernosus bzw. der Plica petroclinoidea anterior wird er von einer dünnen Arachnoidalscheide begleitet.

Der N. abducens tritt durch eine längsovale Pforte im Clivusbereich durch die Dura mater cerebri. Die Entfernung seines Eintrittsortes vom Processus clinoideus posterior ist unterschiedlich. Auch der N. abducens wird von einem engen Arachnoidalspalt eine Strecke weit begleitet.

Sowohl die Eintrittsorte als auch die “intraduralen” Verlaufsstrecken der Nn. oculomotorius, trochlearis und abducens sind zahlreichen Variationen unterworfen (s. Abb. 1–6).

Die Ausformung der Plicae petroclinoideae und der von ihnen erzeugten Wannenregion unterliegen zahlreichen individuellen Schwankungen.
