Zusammenfassung
Nach einer ausführlichen Darstellung des pädiatrischen und chirurgischen bzw. neurochirurgischen Schrifttums über Hirnabscesse bei Kindern im allgemeinen und Kleinhirnabscesse im besonderen wird auf die Vielfalt der anamnestischen Angaben und auf die wechselnden klinischneurologischen und elektrencephalographischen Befunde bei cerebellaren Abscedierungen hingewiesen. Es wird auf diagnostische Täuschungsmöglichkeiten durch interkurrente Besserungen des Allgemeinbefindens, der neurologischen Befunde und des Elektrencephalogramms speziell unter dehydratisierender Therapie aufmerksam gemacht. An zwei Beispielen tödlich verlaufener Kleinhirnabscesse bei Kindern bei denen zunächst an eine Encephalitis oder an ein Rezidiv einer tuberkulösen Meningitis gedacht wurde, werden die differentialdiagnostischen Schwierigkeiten verdeutlicht. Foudroyante Verläufe in der Endphase der Erkrankung verhinderten insbesondere in einem Falle bei bereits bestehendem Verdacht auf einen krankhaften Prozeß der hinteren Schädelgrube die vereinbarte Verlegung in eine Neurochirurgische Klinik. Pathologisch-anatomisch waren beide Kleinhirnabscesse schon von einer relativ festen Membran bzw. Kapsel umgeben.
Summary
After a detailed review of the pediatric and surgical or neurosurgical literature on abscesses of the brain in children generally spoken and in particular on abscesses of the cerebellum, reference is made to the great diversity of anamnestic data and to the changing neurological and electro-encephalographical findings at the clinic in cases of abscesses of the cerebellum. The attention is drawn to the possibility of diagnostic errors due to intercurrent improvements in the general state of health, in the neurological findings, and in the electro-encephalogram, particularly under the influence of a dehydratizing therapy. The differentialdiagnostic difficulties are made clear by two examples of abscesses of the cerebellum in children, which ended fatal. At first sight these two cases were suspicious of encephalitis or a relapse of a tuberculous meningitis. The unexpected rapid and dangerous progress in the final stage of the illness prevented the intended transfer to a neurosurgical hospital; particularly in one of the cases there was a marked suspicion of a brain tumor. Pathologically and anatomically both abscesses of the cerebellum were already enclosed in a relatively strong membrane or capsule respectively.
