Zusammenfassung
Bei 40 adipösen Kindern im Alter von 6–14 Jahren wurden stationär durch eine 400-Caloriendiät und Einschaltung von Perioden absoluten Fastens eindrucksvolle Gewichtsreduktionen erzielt. Die durchschnittlichen tägliche Gewichtsabnahme betrug bei 39 Patienten während des Fastens 0,68 kg. Dieses Therapieregime wurde von den Kindern ebenso gut toleriert, wie es von erwachsenen Adipösen her bekannt ist. Hervorzuheben sind das Fehlen eines Hungergefühles und die zunehmende Bewegungsfreudigkeit wegunsfreudigkeit der vorher trägen Kinder. Ernste Komplikationen wurden bei reichlicher Flüssigkeitszufuhr nicht beobachtet, wenngleich eine negative Stickstoffbilanz während des völligen Nahrungsentzuges unvermeidlich ist. Bei vier Kindern mußte die Fastenperiode nach 3 bis 7 Tagen abgebrochen werden, die anderen hielten sie 10–17 Tage durch. Die Gewichtsabnahmen lagen zwischen 4,4–27,4 kg. Dieses therapeutische Vorgehen ist als Grundlage für eine weitere erfolgreiche ambulante Behandlung gedacht.
Summary
On 40 adipose children there was a marked loss in weight by a diet with 400 calories alternating with periods of complete fasting. The daily loss in weight during fasting was av. 0.68 kg. This regimen of therapy was tolerated by the children as well as known from adults. The missing of orexia and the increasing mobility of the previously inert children have to be accentuated. Grave complications did not occur, if plentiful water was given; a negative balance of nitrogen was during the complete fasting of course unavoidable. On four children the fasting period had to be cut short after 3 to 7 days, the other patients continued 10 to 17 days. The total loss in weight differed between 4.4 and 27.4 kg. A therapy in this way is thought fit to be a base for further successful ambulatory treatment.
