Summary
The complications, which occur in encircling-operations, such as String-Syndrome, scleral Nekrosis, glaucoma-development (due to narrowing the anterior chamber) as well as closure of the central artery most frequently happen, when the globe has been tightened too much.
From the calculations we have made using the cerclage element in reducing the circumference and volume (Fig. 2) we have shown that an identation of the sclera more than 3 mm deep is not advisable. The Silicon-Cerclage-Materials, which we use, are tested by the tension-testing machine, which determines the modulus of elasticity.
The stress, that is force per sectional area, is determined in relation to the amount of elongation in the material (Fig. 3). A round silicon rod with a diameter of 2 mm and a modulus of elasticity between 150 and 200 p/mm2 proved most effective.
In addition, we have conducted experiments with the Cerclage-rods on enucleated human eyes (Fig. 4). In one experiment the intraocular pressure was determined in relation to the pull of the weight upon the globe (Fig. 5). In a second experiment the reduction of the circumference was related to the height of the intraocular pressure (Fig. 6). The elastic Cerclage-rod has the advantage of creeping, when tensed, it tries to regain its original length.
However, the globe, owing to the pressure within it, works against the creeping. With the help of the stress-strain diagram (Fig. 7) and the experiments on the human eye we can determine more accurately the degree of the creeping. In index 1 we can see how much the circumference has been finally reduced including the creeping in relation to the height of the intraocular pressure. The intraocular pressure should not be increased above 35 mm Hg. Otherwise:

the circumference of the globe will be considerably more reduced;

the intraocular pressure will rise much more slowly when compared to results at a lower eye-tension and

3.
the volume reducing-effect will diminish markedly. It is important to measure the intraocular pressure before the end of the operation in order to avoid the above mentioned complications.
Zusammenfassung
Die Cerclage-Operation hat sich besonders in prognostisch ungünstigen Fällen durchgesetzt, obwohl eine Reihe von Komplikationen möglich sind: String-Syndrom, Nekrose der Sklera unter dem eindellenden Material und Sekundärglaukom durch Abflachung der Vorderkammer.
Unsere Studie hat das Ziel, an Hand von klinischen und experimentellen Erfahrungen optimale Bedingungen für die Cerclage-Operation am Auge auszuarbeiten. Anhand von Berechnungen über die Verkürzung des Bulbusumfangs und die Volumenreduzierung (Abb. 2) mit dem Cerclage-Element wird dargelegt, daß eine Skleraeindellung um mehr als 3 mm nicht sinnvoll erscheint. Die von uns benutzten Silicon-Cerclage-Stäbe werden mit der Zugprüfmaschine getestet, die zur Bestimmung des Elastizitäts-Modul dient. Es wird die Spannung, die als die Kraft pro Flächeneinheit definiert ist, in Abhängigkeit zur Dehnung des untersuchten Materials (Abb. 3) ermittelt. Ein runder Silicon-Stab mit einem Durchmesser von 2 mm und einem Elastizitäts-Modul zwischen 150 und 200 p/mm2 hat sich am besten bewährt.
Außerdem werden Experimente mit den Cerclage-Stäben am enucleierten menschlichen Auge vorgenommen (Abb. 4). Es wird einmal der intraoculare Druck in Abhängigkeit vom Gewichtszug auf den Bulbus bestimmt (Abb. 5) und zum anderen die Verkürzung des Bulbusumfanges in Abhängigkeit von der Höhe des intraocularen Druckes gemessen (Abb. 6).
Der elastische Cerclage-Stab hat den Vorteil des Nachzuges, er versucht im angespannten Zustand seine Ausgangslänge zu erreichen, während der Bulbus, bedingt durch den Augeninnendruck, diesem Bestreben entgegenwirkt. Mit Hilfe des Kraftdehnungsdiagramms (Abb. 7) und der Experimente am menschlichen Auge können wir die Höhe des Nachzuges annähernd bestimmen. In Tabelle 1 ist die endgültige Verkürzung des Bulbusumfanges einschlie Blich des Nachzuges in Abhängigkeit zur Höhe des intraocularen Druckes zusammengestellt. Daraus ergibt sich, daß der intraoculare Druck nicht über 35 mm Hg. steigen sollte, weil es dann zu einer wesentlich stärkeren Umfangverkürzung des Bulbus kommt, die Tensio dagegen viel langsamer ansteigt und die Volumenreduzierung deutlich geringfügiger wird. Eine Messung des intraocularen Druckes vor Beendigung der Cerclage-Operation wird daher gefordert, um die Vielzahl von Komplikationsmöglichkeiten zu vermeiden.
