Summary
Manifestation of hereditary spherocytosis in the new-born-period is a rarity, although the number of publications is grown in the last years. The most essential prop of the diagnostic is a positive history of the family besides microspherocytosis and reduced osmotic pressure of the erythrocyts. The leading symptom of this period of life is the hyperbilirubinaemia, which can be accompanied with anaemia and splenomegaly. By the transfusion of exchange a splenectomy in the new-born-period is without object. This fact also is applicable in certain measure in the early infancy, how it is shown with the own observation. Prognosis of the complaint by early manifestation is essentially more unfavourable, and more frequenter bloodtransfusions are necessary. By desferrioxamin you can counteract the hemosiderosis after transfusion.
Zusammenfassung
Die Manifestation der hereditären Sphärocytose in der Neugeborenenzeit gilt als eine Seltenheit, obwohl die Zahl der Publikationen in den letzten Jahren zugenommen hat. Die wesentlichste Stütze der Diagnostik ist außer der Mikrosphärocytose und der herabgesetzten osmotischen Resistenz der Erythrocyten eine positive Familienanamnese. Als Leitsymptom für diesen Lebensabschnitt gilt die Hyperbilirubinämie, die von einer Anämie und Splenomegalie begleitet sein kann. Durch die Austauschtransfusion erweist sich eine Splenektomie in der Neugeborenenperiode als gegenstandslos. Das trifft auch im gewissen Rahmen, wie an der eigenen Beobachtung gezeigt wird, für die Säuglingszeit zu. Die Prognose des Leidens ist bei Frühmanifestation wesentlich ungünstiger und erfordert häufigere Bluttransfusionen. Einer Transfusionshämosiderose kann mit Desferrioxamin entgegengewirkt werden.
