Immunelektrophorese bei psoriatischer Arthropathie

Summary

The serum of 59 psoriatics with roentgenologically ensured arthropathy was investigated by immunoelectrophoresis. Ig _G , Ig _A , Ig _M were determined as well as the titer values of Waaler-Rose-tests and the Rheuma-Latex-fixation. The rheuma tests were negative in all sera examined. Ig _M was found to be decreased in about a half of the sera and increased in a third. It was conspicuous that in the so called microforms of psoriatic arthropathy an increase of the Ig _M -fraction was frequent by seen, where as a decrease was seldom. With severe arthropathy the number of cases with diminished Ig _M -fraction grew, so that in two-third of the patients with visible articular destructions, a reduction of the Ig _M -fraction revealed. An interpretation of the immunoelectrophoretic results in sense of an antibody-deficiency-syndrom is tried to be given in which not only the disturbance of keratinization of the epidermis is pointed out but also the genetically given “inferiority” of the joints and the antibody producing system. The antibody-deficiency-syndrom is getting manifest only when the disease is progressive and is then traceable by semiquantitative methods of immunoelectrophoresis.

Zusammenfassung

Serum von 59 Psoriatikern mit röntgenologisch nachgewiesenen Arthropathien wurde immunelektrophoretisch aufgearbeitet. Bestimmt wurden Ig _G , Ig _A und Ig _M , ferner die Titerwerte des Waaler-Rose-Tests und der Rheuma-Latex-Fixation. Die “Rheuma-Serologie” war bei sämtlichen Patienten negativ. Bei ungefähr der Hälfte der Patienten war Ig _M vermindert und bei einem Drittel vermehrt. Auffallend war, daß bei den sogenannten “Mikroformen” der psor. Arth. häufig eine Vermehrung der Ig _M -Fraktion gefunden würde, während eine Verminderung derselben nur selten festgestellt werden konnte. Mit Verstärkung der Arthropathie kommt es aber vermehrt zu einer Zunahme der Fälle mit verminderter Ig _M -Fraktion, so daß bei zwei Dritteln der Patienten mit sichtbaren Gelenkdestruktionen eine Verminderung der Ig _M -Fraktion nachgewiesen werden konnte. Eine Interpretation der immunelektrophoretischen Befunde im Sinne eines partiellen Antikörpermangel-Syndroms wird versucht, wobei nicht nur auf die genetisch verankerte potentielle Verhornungsstörung der Epidermis, sondern auch auf eine genetisch fixierte “Minderwertigkeit” der Gelenke und des Antikörper-produzierenden Systems hingewiesen wird. Das Antikörpermangel-Syndrom wird erst im Verlauf der schubweisen Erkrankung manifest und durch die semiquantitative Methode der Immunelektrophorese nachweisbar.
