Summary

Immunofluorescence studies have been carried out in 33 cases of disseminate lupus erythematosus (LED), including 11 cases without skin lesions. In 8 cases they were performed several times during treatment with corticosteroids and Imuran. 85 patients with different skin diseases served as a control. Skin specimens were taken from lesions, in 10 cases additionally from the uninvolved skin, as well as in 11 LED cases without skin changes.

In all but one case treated during two years with large doses of corticosteroids the immunofluorescence method demonstrated in vivo fixed immunoglobulins and complement in the epidermodermal junction.

No correlation was established between the ANF-titer and the presence and intensity of the specific fluoresence at the epidermodermal zone. The treatment with corticosteroids diminishes the specific fluorescence, but in majority of cases treated even for several months with low dosages the fluorescence is still present.

The authors stress the importance of the immunopathologic investigations in the diagnosis of LED.

Zusammenfassung

Es wurden 33 Patienten mit Lupus erythematodes disseminatus (LED) untersucht, davon 11 ohne Hautveränderungen. 8 Fälle wurden während der Behandlung mit Corticosteroiden und Imuran mehrmals untersucht. 85 personen mit verschiedenen Hautkrankheiten stellten das Kontrollmaterial dar.

Die Untersuchungen wurden mittels der Immunfluorescenzmethode in den Biopsien aus den Krankheitsherden, in 10 Fällen zusätzlich auch aus der gesunden Haut, und in 11 LED-Fällen ohne Hautveränderungen durchgeführt.

In allen Fällen, mit Ausnahme eines Falles, der 2 Jahre lang mit großen Dosen von Corticosteroiden behandelt worden war, wurde das Vorhandensein von in vivo gebundenen Immunglobulinen und von Komplement an der epidermo-dermalen Grenze festgestellt.

Die Vergleichsuntersuchungen zwischen dem ANF-Titer und dem Auftreten sowie der Intensität der spezifischen Fluorescenz an der epidermo-dermalen Grenze ergaben keine Korrelation.

Die Behandlung mit Corticosteroiden kann die spezifische Fluorescenz abschwächen, die Negativisierung der Ergebnisse wird aber bei kurzdauernder Verabreichung kleiner Dosen nicht erzielt.

Die Autoren unterstreichen den großen Wert der immunpathologischen Methode für die Diagnostik des LED.
