Summary

The pathogenesis of diabetic retinopathy and the theoretical foundations of different methods of its phototherapy are reviewed. One aim of the treatment may be seen in the destruction of comparatively large retinal areas of small functional value, preferably in the periphery. Another aim of equal importance is the obliteration of the pathological vessels themselves. The two methods may also be combined.

At present we do not know exactly which of the two principles should be preferred in a given case of diabetic retinopathy. It should also be kept in mind that these methods are not sharply differentiated from each other because strictly selective treatment is not possible with available equipment. Our own strategy and the apparatus used are described.

Zusammenfassung

Es wird einleitend auf die Pathogenese und die theoretischen Grundlagen der verschiedenen Methoden der Phototherapie der diabetischen Retinopathie eingegangen.

Während im Prinzip ein Behandlungsziel in der weitgehenden Zerstörung von Netzhaut erblickt werden kann, und zwar vorzugsweise peripherer Gesichtsfeldareale von geringerer funktioneller Wertigkeit, ist ein ebenso wichtiges Ziel die Zerstörung der Netzhautgefäße selbst. Die beiden Methoden können auch kombiniert werden.

Wir wissen zur Zeit noch zu wenig genau, welchem der beiden Behandlungsprinzipien bei einer definierten Form der diabetischen Retinopathie der Vorzug gegeben werden soll. Im übrigen unterscheiden sich die beiden Behandlungsmethoden nur graduell, da es mit den zur Zeit verfügbaren Apparaten nicht möglich ist, streng selektiv zu arbeiten. Die von uns angewandte Behandlungsstrategie und die verwendeten Apparate werden beschrieben.
