Zur nosologischen Stellung der Psoriasis arthropathica

Zusammenfassung

Über die nosologische Stellung der Psoriasis arthropathica besteht bisher nur dahin gehend eine Übereinstimmung, daß der Zusammenhang mit der erblichen Grundkrankheit Psoriasis im Sinne einer Extremvariante gewahrt ist. In seiner Symptomatologie weist das Krankheits-bild aber verschiedene Beziehungen zur primär chronischen Polyarthritis, in zweiter Linie auch zum Morbus Reiter und zur Spondylitis ankylopoetica auf. Daher hat die vorliegende Arbeit eine klare nosologische Präzisierung der Begriffsbestimmung Psoriasis arthropathica unter dermatologischen und rheumatologischen Gesichtspunkten zum Ziel. Die deutlich über der zufälligen Kombinationsquote von Psoriasis und primär chronischer Polyarthritis liegende Häufigkeit des Krankheitsbildes sowie zahlreiche Unterschiede gegenüber der primär chronischen Polyarthritis hinsichtlich der Geschlechts-und Altersverteilung, der klinischen Symptomatologie und des rheuma-serologischen Status lassen eine sichere Abgrenzung gegenüber dem echten Rheumatismus zu. Andererseits wird die nosologische Eigenständigkeit auch noch durch häufige Änderungen im dermalen Erscheinungsbild der Psoriasis unterstrichen. Besonders deutlich sind die klinischen Unterschiede zur Reiterschen Krankheit, mit der nur hinsichtlich der Altersverteilung des Krankheits-beginns, des relativ häufigen Befalls der Ilio-Sacralgelenke und der meist negativen Rheumaserologie Übereinstimmung besteht. Die Unterscheidung eines “klassischen” und eines “rheumatoiden” Typs der Psoriasis arthropathica erscheint uns insofern noch verfrüht, als beweiskräftige Vergleichsuntersuchungen zwischen beiden Typen bisher fehlen und negative rheuma-serologische Agglutinationsteste für sich allein noch keine nosologische Identität begründen. Daher kann zunächst nur der “klassischen”, die distalen Interphalangealgelenke bevorzugenden Psoriasis arthropathica eine nosologische Sonderstellung zuerkannt werden. Diese schließt die Möglichkeit naher genetischer Beziehungen zum Formenkreis der primär chronischen Polyarthritis, der Spondylitis ankylopoetica und des Morbus Reiter nicht aus, da auch bei diesen (zumindest bei den beiden erstgenannten) selbständigen Krankheiten ebenso wie bei der Psoriasis arthropathica hereditäre Einflüsse als ätiologische Teilfaktoren bekannt sind.
