Primary laryngoplasty

Summary

A new procedure is described for the surgical reconstruction of the larynx following total laryngectomy. Postoperatively, the voice was as good as that resulting from the procedure of Asai. Aspiration of fluid into the trachea was minimal. The procedure may be briefly summarized as follows:

  1. 1.

    The lateral portion (about one-half of the total) of both thyroid cartilages are left in situ;

  2. 2.

    a posterior laryngeal wall is fashioned from the superior horns of the same cartilages;

  3. 3.

    the inner surface of the reconstructed larynx is lined by mucosal flaps from the hypopharynx; and

  4. 4.

    the reconstructed larynx is firmly sutured to the base of the tongue.

This procedure was employed in twelve cases with extensive internal laryngeal carcinoma. In three of them, healing was retarded by the formation of postoperative pharyngeal fistulae. All patients regained their voices and their abilities to swallow.

Zusammenfassung

Ein neues Operationsverfahren zur Wiederherstellung des Larynx nach totaler Laryngektomie wurde entwickelt. Die postop. Stimme war ebenso gut wie nach Asais Laryngoplastik. Die Aspiration von Flüssigkeit in die Trachea war nur geringfügig. Das Operationsverfahren des Verfassers ist im folgenden zusammengefaßt:

  1. 1.

    Die lateralen Hälften der Schildknorpelplatten werden auf beiden Seiten belassen;

  2. 2.

    die hintere Larynxwand wird mit Hilfe deroberen Schildknorpelhörner gestaltet;

  3. 3.

    die Innenfläche des wiederaufgebauten Larynx wird mit Stiellappen aus der Hypopharynxschleimhaut gedeckt;

  4. 4.

    der wiederhergestellte Larynx wird am Zungengrund fest vernäht.

Dieses Verfahren wurde in 12 Fällen mit ausgedehntem inneren Kehlkopfkrebs durchgeführt. In 3 Fällen trat eine Verzögerung der Heilung durch Entwicklung einer Pharynxfistel nach der Operation ein. Alle Patienten gewannen ihre Stimme und Schluckbewegung wieder.
