Rapidly developping scoliosis in an adolescent due to spinal osteoblastoma

Summary

A benign osteoblastoma of the spine should be considered a possible cause of painful scoliosis, especially in young adults. This report concerns a 17 year old patient with a lesion of the second lumbar vertebra where her painful progressive scoliosis was mistaken as idiopathic. Considering pain as a common presentation for the osteoblastoma, the diagnosis can be made early through X-rays, bone scan and computed tomography. The only adequate therapy is total removal of the tumor and the surgeon should make every effort to resect this lesion in sano to acheave permanent relief of pain and full alignment of the spine.

Zusammenfassung

Das benigne Osteoblastom der Wirbelsäule sollte immer als mögliche Ursache für die Entstehung einer schmerzhaften Skoliose, besonders im 2. Wachstumsschub in der Adoleszenz berücksichtigt werden. Im folgenden Aufsatz wird der Fall einer 17jährigen Patientin mit einer Läsion des 2. LWK geschildert, deren schmerzhafte progrediente Skoliose als idiopathisch fehlinterpretiert wurde. Jede schmerzhafte, progrediente skoliotische Zwangshaltung in der 2. Lebensdekade ist verdächtig auf Osteoblastom. Gerade die charakteristischen Symptome; starke Schmerzen mit nächtlicher Verschlimmerung sollte Anlaß zur frühzeitigen röntgen- und skelettszintigraphischen Untersuchung sowie der gezielten Computertomographie Anlaß geben. Als einzige adäquate Therapie ist unbedingt eine radikale Exstirpation des Osteoblastoms in sano anzustreben, mit dem Ziel der Schmerzbefreiung und der postoperativen Spontanbegradigung der Wirbelsäule, wie in diesem Fall demonstriert wird.
