Abstract
Since 1969 leucine sensitive hypoglycemia has been diagnosed in 4 infants. Elevated serum insulin levels suggested hyperinsulinism; in 1 infant islet cell hyperplasia was demonstrated by morphological examination of pancreatic tissue.
All cases were treated with diazoxide and have been followed under therapy for periods ranging from 4 months to 6/14 years. The upper limit of dosage was 15 mg/kg.
The blood glucose values promptly became normal in 2 infants under this regimen. The development of these patients was normal. In the 2 other infants treatment was effective only after the additional restriction of daily protein intake to 2–2.5 g/kg. Severe cerebral damage occured in one of these infants due to recurrent episodes of hypoglycemia, causing further development to be considerably restricted; the other one developed satisfactorily but with moderate mental retardation. Both children are suffering from cerebral convulsions. In all patients hypertrichosis was the only side effect.
Zusammenfassung
Seit 1969 wurde bei 4 Kindern die Diagnose einer leucinempfindlichen Hypoglykämie gestellt. Erhöhte Seruminsulinspiegel wiesen auf einen Hyperinsulinismus hin; bie 1 Kind ergab die morphologische Untersuchung von Pankreasgewebe eine Inselzellhyperplasie.
In allen Fällen wurde Diazoxid zur Behandlung eingesetzt. Die Beobachtungsdauer unter Therapie beträgt nunmehr 4 Monate bis 6/14 Jahre. In der Dosierung wurde eine obere Grenze von 15 mg/kg eingehalten.
Diese Therapie führte bei 2 Kindern prompt zu einer Normalisierung der Blutglucose-werte; die Entwicklung dieser Kinder verlief normal. Bei den 2 anderen Kindern war ein ausreichender Behandlungserfolg nur durch eine zusätzliche Beschränkung der täglichen Eiweiß-zufuhr auf 2–2,5 g/kg zu erreichen. Durch rezidivierende Hypoglykämien hat eines dieser Kinder einen schweren Cerebralschaden davongetragen so daß die weitere Entwicklung erheblich beeinträchtigt war; das andere zeigte eine zufriedenstellende Entwicklung bei mäßiggradiger geistiger Retardierung. Beide Kinder leiden an einem cerebralen Anfallsleiden. Als einzige Nebenwirkung trat bei allen Kindern eine Hypertrichosis auf.
