Abstract
A three weeks old girl had a considerably elevated proportion of methemoglobin (18% of total hemoglobin) in the couse of diarrhea. During the following three months there were repeated episodes of diarrhea and these were always accompanied by elevated levels of methemoglobin, sometimes up to 50% of total hemoglobin. The activity of the NADH-dependent methemoglobin reductase in the erythrocytes of the infant was diminished. Only 50% normal activity were found. A heterozygous enzyme defect is presumed. Thirteen members of the infant`s family were found to be heterozygotes. The combination of an heterozygous enzyme defect with a severe methemoglobinemia is extremely rare. In our patient a toxic substance presumably developing during diarrhea may have produced large amounts of methemoglobin. Because newborns only have 50% adult methemoglobin reductase-activity the association with a reductase defect will not allow sufficient reduction of the produced methemoglobin.
Zusammenfassung
Bei einem weiblichen Säugling kam es im Alter von 3 Wochen im Verlaufe einer Dyspepsie akut zu einer erheblichen Methämoglobinämie von 18% des vorhandenen Hämoglobins. In den folgenden Monaten traten wiederholt dyspeptische Schübe auf, die jeweils mit einem Anstieg der Methämoglobin-Konzentration, zeitweise bis 50%, verbunden waren. In den Erythrocyten des Kindes wurde eine auf 50% des Normalwertes verminderte Aktivität der NADH-abhängigen Methämoglobin-Reduktase gefunden. Offensichtlich handelt es sich um einen heterozygoten Enzymdefekt. Bei der Untersuchung der Familie wurden 13 weitere heterozygote Defektträger beobachtet. Das Auftreten einer schweren Methämoglobinämie ist bei heterozygotem Reduktasemangel sehr selten. Bei unserer Patientin hat vermutlich eine während der Dyspepsien entstandene Noxe zur vermehrten Bildung von Methämoglobin geführt. Da Neugeborene nur 50% der Methämoglobin-Reduktase-Aktivität von Erwachsenen haben, kommt es bei einem gleichzeitigen Methämoglobin-Reduktase-Defekt nicht zu einer ausreichenden Reduktion des entstandenen Methämoglobins.
