Zusammenfassung

Die Galaktosämie ist eine häufig zum Tode führende, angeborene, erbliche Stoffwechselstörung, die auf einem Mangel der die Galaktose verarbeitenden Fermente beruht. Die Krankheitserscheinungen werden durch toxische Wirkung der Galaktose hervorgerufen; sie treten meist sofort nach Beginn der Milchfütterung bei jungen Säuglingen auf. Das klinische Bild ist gekennzeichnet durch schwere Ernährungsstörung mit Erbrechen und Durchfällen, Dystrophie, Zuckerausscheidung, Ikterus, Leber- und Milzvergrößerung, Katarakte und Entwicklungsrückstand.

Zuckerausscheidung bei mit Milch ernährten Säuglingen ist stets auf Galaktosämie verdächtig; die Untersuchung des Harns auf Zucker ist deshalb bei Auftreten der genannten Krankheitserscheinungen unerläßlich. Die Diagnose wird durch die Identifizierung des ausgeschiedenen Zuckers gesichert.

Die Behandlung besteht lediglich in dem Fortlassen des Milchzuckers aus der Nahrung; rechtzeitig eingeleitet gewährleistet sie die normale Entwicklung der Patienten.

Es werden zwei eigene Beobachtungen von Galaktosämie bei einem 4 Monate alten und einem 5 Wochen alten Säugling mitgeteilt. Zwei weitere Fälle, in der Neugeburtsperiode verstorbene ältere Geschwister eines der Patienten betreffend, bei denen die Diagnose nachträglich gesichert erschien, werden kurz besprochen.

Für die Bestimmung der Glucose und Galaktose im Blut wird eine Methode angegeben, bei welcher die Gesamtreduktion vor und nach Entfernung der wahren Glucose durch Glucose-Oxydase bestimmt wird; die Vorteile dieses Verfahrens gegenüber der Blutzuckerbestimmung vor und nach Vergärung mit Hefe werden hervorgehoben.

Ätiologie und Pathogenese, Diagnose und Differentialdiagnose sowie die Therapie der Erkrankung werden an Hand der Literatur und der eigenen Beobachtungen besprochen.
