Bilaterale Nierenrindennekrosen im Rahmen eines Sanarelli-Shwartzman-Phänomens nach Pneumokokkenmeningitis bei chronisch-essentieller Erythroblastopenie (sogenannte Blackfan-Diamond-Anämie)

Zusammenfassung

Es wird über den Verlauf einer chronisch-essentiellen Erythroblastopenie (sog. Blackfan-Diamond-Anämie) bei einem 4 Jahre, 11 Monate alten Mädchen berichtet. Dabei erweckten die lange Verlaufsdauer, die Therapie sowie deren Komplikationen besonderes Interesse, wobei wir der interkurrenten Meningokokken-Meningitis sowie der tödlichen Pneumokokkenmeningitis in Verbindung mit Anurie, hämorrhagischer Diathese und Kreislaufkollaps besondere Bedeutung beimaßen. Pathologisch-anatomisch wurden Organveränderungen gefunden, die sich einem Sanarelli-Shwartzman-Phänomen zuordnen ließen. Dabei standen bilaterale Nierenrindennekrosen, die Verstopfung kleiner Nierenrindengefäße sowie der Glomeruluscapillaren durch sog. Fibrinthromben im Vorder-grund. Gleichen Gefäßinhalt sahen wir im Gehirn, der Leptomeninx und in den Sinusoiden der Leber. Wir glauben, die pathologisch-anatomischen Befunde im Rahmen eines SSP mit der hochfieberhaften Pneumokokken-meningitis mit Anurie und der vorangegangenen Cortisontherapie bzw. der stark ausgeprägten Hämosiderose in Verbindung bringen zu können.

Summary

Report relating to the course and the complications of chronic essential erythroblastic anemia (the so-called Blackfan-Diamond anemia) with special reference to the importance of its therapy, the intercurrent meningococcic meningitis and the pneumococcic meningitis in connection with anuria, hemorrhagic diathesis and circulatory collapse. The pathologic-anatomical examination showed organic transformations, e.g. bilateral renal cortex necrosis and the so-called fibrin thrombus in small renal arteries, glomerulic capillaries, capillaries of the liver, cerebral and leptomeninges capillaries, which could be associated with the Sanarelli-Shwartzman phenomenon. In our opinion, the pathologic anatomical diagnosis is immediately connected with the highly febrile pneumococcic meningitis with anuria and the preceding cortison therapy resp. the hemosiderosis.
