Summary
Flexion and extension radiographs in myelography in preoperative spondylolisthesis patients were studied.
Dural sac motion at the level of the mid-vertebral bodies was essentially anterior in extension. At the level of the intervertebral disc, the motion is obstructed by the disc bulging posteriorly in extension.
Axial motion is variable and not linearly correlated with spinal column length change. The dural sac undergoes a localized narrowing at the level of the spondylolisthesis. This becomes more prominent during extension. In 21 cases studied completely, no disc herniation was encountered myelographically of surgically.
The anterior epidural space is of a greater value at the level of the spondylolisthesis than reported normal values. The change in this space from dorsiflexion to ventral flexion is much less at the level of olisthesis than either the adjacent areas of a comparable adjacent intervertebral level.
Zusammenfassung
Aus einem Kollektiv von 21 Patienten mit Spondylolisthesis wurden präoperativ Funktionsröntgen-Aufnahmen während der Myelographie ausgewertet.
Die Dursalsack-Bewegung auf der mittleren Wirbelkörper-Höhe ist in der Extensionsbewegung wesentlich weiter vorn; in Höhe der Intervertebral-Bandscheibe wird die Bewegung durch die sich nach dorsal verlagernde Bandscheibe in der Extension behindert.
Die Axial-Bewegung ist variabel und nicht in Linear-Korrelation mit der Änderung der Wirbelsäulen-Länge. Der Duralsack unterliegt einer lokalisierten Verengung in Höhe der Spondylolisthesis; während der Extension wird dieses deutlicher.
In 21 komplett ausgewerteten Fällen wurde weder myelographisch noch chirurgisch ein Bandscheibenvorfall gefunden. Der vordere epidurale Raum ist in Höhe der Spondylolisthesis von größerem Wert als berichtete normale Werte.
Die Bewegungsänderung von der Dorsal-Flexion zur Ventral-Flexion ist auf der Höhe der Olisthesis deutlicher geringer als in den angrenzenden vergleichbaren Intervertebral-Höhen.
