The treatment of fractures of the proximal humerus

Summary

Fractures of the proximal humerus, which can not be kept in place after reposition—and therefore not acceptable for conservative treatment—can be sufficiently fixed by a thin and long Rush-pin percutaneously introduced. So the damaged arm can be moved early. No inflammation and no pseudarthrosis was observed in 124 cases. The follow up examination of 98 patients after 6 to 12 months revealed good results. In 4 children the fractured neck of the humerus could not be reposed into a proper position (Interposition of periosteum and joint capsule). After open reduction fixation was performed by Kirschner's wires. We observed fast and complete healing in all cases.

Zusammenfassung

Brüche am proximalen Ende des Humerus, die sich nicht retinieren lassen und deshalb für die reine konservative Behandlung ungeeignet sind, lassen sich durch perkutane Einführung eines dünnen, langen Rushpins ausreichend fixieren. Es kann frühzeitig mit funktioneller Behandlung begonnen werden. Bei 124 Patienten mit solchen Frakturen gab es keine Infektion und keine Pseudarthrose. Bei 98 Nachuntersuchten (65 Patienten älter als 60 Jahre, 33 Patienten jünger als 60 Jahre) ergaben sich nach 6–12 Monaten gute Ergebnisse. Bei 4 irreponiblen kindlichen Brüchen (Interposition von Periost und Gelenkkapsel) wurde die Fraktur nach Eröffnung und Reposition mit Kirschnerdrähten gespickt. In allen Fällen kam es zur raschen und völligen Ausheilung.
