Summary
A rotatory test is described for a quick assessment of vestibular function. The patient, seated on a rotating chair, is first subjected to a constant angular velocity. Then, acceleration of 3.0°/sec2 is applied for a duration of 30 sec, to be followed once more by constant velocity (90°/sec) for another 180 sec. Thereafter the chair is stopped abruptly, i.e., its velocity is decreased to zero within 0.3 sec. The maximal beat rate of the per-rotatory nystagmus during the 30-sec period of maximal nystagmus is plotted. Furthermore, the beat rate of the post-rotatory nystagmus is integrated over the first 30 sec. It is plotted on double-logarithmic coordinates which render a straight line characteristic. The four characteristics, connected on a chart, exhibit two L-shaped patterns each for the nystagmus in either direction. The position on the chart and the shape of the two L-figures may be evaluated by pattern recognition in a way similar to that used in the inspection of audiograms.
Zusammenfassung
Die vorliegende Arbeit beschäftigt sich mit einer neuartigen gemeinsamen Darstellung der Frequenzauswertung der per- und postrotatorischen Nystagmus I. Es wird ein überschwelliges rotatorisches Stimulusmuster mit 3,0°/sec2. Beschleunigung, 90° Endgeschwindigkeit und abruptem Abbremsen aus der Endgeschwindigkeit benutzt. Die maximale per- und postrotatorische Reaktionsdynamik der Nystagmusfrequenz wird als gerade Kennlinie in einem L-förmigen Testschema zur Darstellung gebracht. Normbereiche eines Patientenkollektivs von 90 unausgelesenen Patienten werden in dieses Schema eingetragen. Acht typische Musterkombinationen gehen aus dem Test hervor. Sie ermöglichen dem Betrachter, prima vista die audiogrammanaloge mustererkennende Diagnostik. Hinsichtlich des postrotatorischen Frequenzverhaltens ist bemerkenswert, daß die Zuwachsfunktion sowohl in Einzelfällen als auch für den Mittelwert und die Standardabweichung eine erstaunliche Gradlinigkeit aufweist, wenn sie nach Art einer Potenzfunktion im doppellogarithmischen Feld von Zeit und Nystagmusschlagsumme aufgetragen wird.
