Summary
The medium gluteal muscles, the adductor and iliopsoas muscles were examined by electromyography on 42 patients, 3 months to 8 years following arthrodesis of the hip joint.
Concurring with general clinical observations, we found a disuse atrophy of these muscles, though relatively scarcely marked and most noticable with the iliopsoas muscle.
The function of the hip muscles examined in upright standing corresponds exactly with the static conditions on the operated side, depending on the position of the immobilised joint, weight bearing, potential and balance.
During ambulation a tensilerhythm was electromyographically demonstrable, corresponding without noticable difference with the non operated side.
The results of examination obtained offer contain consequences for the operative technique and follow-up care of hip joint arthrodesis.
Résumé
Chez 42 malades fut procédé, de trois mois à huit ans après arthrodése de la hanche, un examen électromyographique des muscles moyen fessier, adducteurs de la cuisse et psoasiliaque.
On constata, en accord avec les données cliniques, une atrophie d'inactivité relativement restreinte de ces muscles, marquée surtout au niveau du psoas-iliaque.
La fonction des muscles de la hanche examinée en position debout correspond exactement aux données statiques dépendant de la position de l'articulation fixée, de la portance et de la stabilité du côté opéré.
Pendant la marche on mit en évidence à l'électromyogramme un rythme de contraction isométrique ne différant pas de celui enregistré au niveau de l'articulation d'une hanche normale.
Des résultats de ces expérience on peut déduire des conséquences précises quant à la technique opératoire et les soins post-opératoires des l'arthrodése de la hanche.
Zusammenfassung
Es wurden bei 42 Patienten 3 Monate bis fast 8 Jahre nach operativer Versteifung eines Hüftgelenkes die Mm. glutaeus medius, adductores und iliopsoas elektromyographisch untersucht.
In Übereinstimmung mit den allgemeinen klinischen Beobachtungen ergab sich eine, allerdings relativ gering ausgeprägte Inaktivitätsatrophie dieser Muskeln, welche am Iliopsoas am deutlichsten ist.
Die Funktion der untersuchten Hüftmuskeln beim Stehen entspricht genau den von der Stellung des fixierten Gelenkes, Belastbarkeit und Standsicherheit auf der operierten Seite abhängigen statischen Verhältnissen.
Während des Gehens ließ sich elektromyographisch ein Anspannungsrhythmus nachweisen, der ohne erkennbaren Unterschied dem bei einem gesunden Hüftgelenk entsprach.
Aus den erhaltenen Untersuchungsergebnissen lassen sich bestimmte Konsequenzen für die operative Technik und Nachbehandlung der Hüftarthrodese ableiten.
