Die Arthrodese des Handgelenkes

Summary

After a short description of the anatomy and function of the wrist the own 22 cases of arthrodesis of this joint in 21 patients are analysed. The main indications were: radiocarpal arthrodesis due to lunatummalacia (8), non-union of the navicular bone (6), and flexion contracture in spastic hemiplegia (3). 21 times an intraarticular arthrodesis was performed and once an extra-articular one. 15 times the inlay-method with tibia or radius grafts was performed and once the onlay-method with a tibia graft. Further methods of arthrodesis were: pressure-osteosynthesis by spongiosascrews (4), graft and screws (1) and primary as well as secondary osteosynthesis by plates (1 case each). There was a formation of pseudarthrosis twice and once a resorption of the distal part of the graft which, after 8 years led to pains due to loosening of the arthrodesis, and once there was an infection. The partial arthrodesis had the subjectively worse results: 2 patients with radiocarpal arthrodesis complained of pains, but only one of the patients with arthrodesis up to the distal carpal row; in no case, however, a loosening of the carpometacarpal joints was to be found. If certain aspects of operationel technique are observed — as, for instance, a scrupulous removal of the cartilage, a sufficiently strong graft, a saft latch, an avoidance of postoperative changes of position — the arthrodesis of the wrist with grafts taken the same organism is a safe and effective operational process, in which the application of additional material for osteosynthesis can be dispensed with, setting aside cases of excessive osteoporosis in rheumatoid arthritis.

Zusammenfassung

Nach kurzer Schilderung der Anatomie und Funktion des Handgelenkes wird das eigene Krankengut von 22 Arthrodesen dieses Gelenkes bei 21 Patienten analysiert. Hauptindikationen waren radiokarpale Arthrosen infolge Lunatummalacie (8) und Navicularpseudarthrose (6) sowie Beugekontrakturen bei spastischer Hemiplegie (3). 21mal wurde eine intra- und 1mal eine extraartikuläre Versteifung ausgeführt. 15mal kam die Inlaytechnik mit Tibia- oder Radiusspänen und einmal die Onlaytechnik mit einem Tibiaspan zur Anwendung. Weitere Arthrodesenmethoden waren: Druckosteosynthese mit Spongiosaschrauben (4), Verspanung und Verschraubung (1) und primäre oder sekundäre Plattenosteosynthese (je 1). 2mal kam es zur Pseudarthrosenbildung, 1mal zur Resorption des distalen Spananteiles, so daß nach 8 Jahren infolge Lockerung der Arthrodese Schmerzen auftraten, und 1mal zur Infektion. Die partiellen Arthrodesen brachten die subjektiv schlechtesten Ergebnisse: von 3 radiokarpalen Versteifungen klagten 2 über Schmerzen, von 9 Arthrodesierungen bis zur distalen Handwurzelreihe dagegen nur 1 Patient, wobei in keinem Fall eine Lockerung der Carpometakarpalgelenke gefunden wurde. Die Handgelenksarthrodese mit autologen Knochenspänen ist, wenn bestimmte operationstechnische Gesichtspunkte (z. B. möglichst exakte Entknorpelung, genügend großer Span, sichere Verankerung, Vermeiden von postoperativen Stellungsänderungen) beachtet werden, ein sicheres und leistungsfähiges Operationsverfahren, so daß, von Ausnahmesituationen abgesehen (z. B. hochgradige Osteoporose bei rheumatoider Arthritis), auf das Einbringen von zusätzlichem Osteosynthesematerial verzichtet werden kann.
