Die Querschnittslähmung bei Mehrfachverletzungen

Zusammenfassung

Es wird über die Erfahrungen in der Behandlung von Querschnittslähmungen bei schweren Mehrfachverletzungen berichtet. Auch wenn die Überlebenschancen gering sind, sollen sämtliche Verletzungen primär definitiv versorgt werden. In der Dringlichkeit der Versorgung stehen die Verletzungen des Schädels und der großen Körperhöhlen an erster Stelle. Frakturen an den Extremitäten sollen primär operativ stabilisiert werden. Hingegen sind operative Eingriffe an der Wirbelsäule primär nicht angezeigt. Die Prognose ist infaust bei Verletzungen im höheren Alter und bei hohen Querschnittsläsionen. Von 9 Verletzten konnten nur 2 erfolgreich rehabilitiert werden.

Summary

This report concerns experiences in the treatment of transverse lesions of the spinal cord as part of severe multiple injuries in one same patient. Though chances for survival are poor, all injuries should be taken care of in a definitive manner. Among the various injuries, those of the skull and body cavities have first priority for surgical care. Fractures of extremities need to be stabilized on first attempt. Operative interventions on the spinal cord have no priority in the presence of other life endangering injuries. Prognosis is garded in patients of the higher age groups and with high cord lesions. Of 9 such cases with severe multiple injuries only 2 could be successfully rehabilitated.

Résumé

Ce report présente des expériences sur le traitement des blessûres multiples avec des lésions transversales de la médulle èpinale. Les chances pour survivre are minimales. Bientôt, toutes les blessures doivent etre traité definitivement. L'ordre de la priorité commence chez les lésions craniennes, et chez les blessûres des cavitès du tronc. Les fractures des extrémitées doivent etre stabilisé en première attempt therapeutique. Les lesion du medulle épinale n'ont pas de la priorité. La prognose est très mauvais, especialement chez les malades agées et chez des lésions medullaires hautes. Il n'etait possible de rehabiliter que 2 de 9 blessés avec succes.
