Zusammenfassung
Bei drei Unterschenkelfrakturen wurden Wundsekret und Serum nach Infusionen von 10 Mill. E Penicillin G auf ihre Antibioticakonzentrationen untersucht. Es fanden sich hohe, für viele Erreger bactericide Spiegel über mehr als 2 Std. Die Konzentrationen im Wundsekret entsprachen weitgehend den Serumspiegeln. Das Wundsekret ist somit in der Lage, Antibiotica an durchblutungsgestörtes Gewebsmaterial zu bringen.
Antibiotische Prophylaxe wird bei Knochenoperationen im allgemeinen abgelehnt. Eine Ausnahme bilden offene Knochenbrüche, Frakturen mit schweren Weichteilschäden und Hautschädigungen sowie Brüche, bei deren Versorgung ausgedehnte plastische Weichteileingriffe erforderlich sind.
Summary
Following administration of 10 mill. units of Penicillin G intravenously its concentration in the serum and the wound secretions of three patients with shant fractures was examined. There were high levels of bactericidal concentration for more than 2 hours. The concentration in the wound secretions equals the serum concentrations. Wound secretions seemed to be a sufficient transportation medium for antibiotics within tissue the circulation of which is short of normal.
The prophylactic use of antibiotics in bone surgery is not recommended unless there are complicated fractures, fractures combined with severe skin or tissue damage and fractures requireing extensive reconstructive or plastic procedures.
Résumé
Lors de fractures de jambe, on a examiné après perfusions de 10 millions U.I. de Pénicilline G, la concentration simultanée d'antibiotiques au niveau du sérum et au niveau des sécrétions opératoires. Les taux trouvés furent très élevés et bactéricides et cependant 2 heures au moins. Les concentrations au niveau des sécrétions opératoires étaient très analogues à celles correspondantes du sérum. Les sécrétions opératoires sont donc capables d'apporter au niveau des tissus lésés mal irrigués des doses bactéricides d'antibiotiques. En général on ne fait pas d'antibiothérapie préventive dans la chirurgie osseuse. Une exception est faite pour les fractures ouvertes ou bien avec grands délabrements viscéraux et cutanés en regard du foyer de fracture. Dans ces cas là l'administration rapide et discontinué de hautes doses de Pénicilline G est conseillée.
