Idiopathische Fußdeformitäten

Zusammenfassung

Es wurde die Pathogenese der idiopathischen Fußdeformitäten (Hallux valgus und Hammerzehe) an Hand histologischer Schnitte der Arterien, Nerven und Fußmuskeln bei 42 Probanden beiderlei Geschlechts untersucht; das Alter lag zwischen 12 Tagen und 90 Jahren. Vom 7. Lebensjahr an fanden sich muskuläre Schädigungen in Form von Ischämie und Denervation. Der Verfasser ist der Auffassung, daß diese Schädigungen das Kräftegleichgewicht der Muskeln stören und diese Tatsache die Verbreiterung des Vorfußes und das Absinken des Quergewöbes nach sich zieht. In der Folge treten dann die Deformitäten Hallux valgus und Hammerzehe auf.

Die ersten Veränderungen fanden sich bei Kindern von 3 Jahren an den kleinen Arterien. Zeitlich gesehen folgen nach den arteriellen Schädigungen die der Nerven und als letztes die der Muskeln. Erstere sind wahrscheinlich auf die aufrechte Haltung des Menschen zurückzuführen. Somit sind also die idiopathischen Fußdeformitäten Hallux valgus und Hammerzehe der Preis. den der Mensch für die von ihm erworbene Haltung zu zahlen hat.

Der Verfasser beschreibt eine neue Form des chirurgischen Eingriffes, die zur Korrektur des Hallux valgus beim jungen Menschen erarbeitet wurde.

La pathogenie des deformations idiopathiques du pied humain («hallux valgus» et «orteils en griffes») a été étudié a travers l'examen histologique des artères, des nerfs et des muscles intrinsèques des pieds de 42 individus des deux sexes et d'âge compris entre 12 jours et 90 ans. A partir de l'âge de 7 ans on a rencontré des lésions musculaires d'ischaemie et de dénervation. L'auteur pense que ces lésions entraînent un désequilibre des forces musculaires et que ce désequilibre est responsable de l'élargissement de l'avant pied et de la chute de l'arche métatarsienne transversale. En second lieu s'installent les deformations de l'«hallux valgus» et de «orteils en griffes».

Les lésions les plus précoces ont été recontrées dans les artères de moindre épaisseur, chez des enfants de 3 ans. Les lésions nerveuses apparaissent après les lésions artérielles et en dernier lieu apparaissent les lésions musculaires. Les lésions artérielles semblent être dues à la posture verticale. Ainsi, les deformations idiopathiques de «hallux valgus» et des «orteils en griffes» sont le prix que l'être humain paye pour la posture qu'il a acquise.

L'auteur décrit une nouvelle forme d'intervention chirurgicale qu'il a mise au point pour la correction de «hallux valgus» sur les jeunes.

Summary

The pathogenesis of the so called idiopathie deformities of the human foot “hallux valgus” and “claw toes” was investigated through histological studies of the arteries, nerves and intrinsic muscles of the feet of 42 subjects of both sexes, from the age of 12 days to 90 years. Lesions due to ischaemia and denervation were found in these muscles, from the early age of 7 years onwards. The Author thinks that these lesions are responsible for the loss of muscle balance that leads to splaying of the fore foot, to the fall of the transverse metatarsal arch and secondarily to “hallux valgus” and “claw toes”.

The earliest lesions were found in the finer arteries of the feet of children 3 years old. The nerve lesions follow the arterial ones and the muscular come last. These are found at the early age of 7. The arterial lesions are similar to those found in the limbs of the giraphe and in benign hypertension.

They are probably due to excessive hydrostatic pressure related with the erect human posture. Therefore idiopathic “hallux valgus” and “claw toes” are only one of the tribute man has to pay for his posture.

The Author describes a new operation for the correction of “hallux valgus” before arthritic changes have taken place at the metatarso-phalangeal joint: surgical synostosis between the first and the second metatarsal with medial capsulotomy and transplantation of the long extensor of the second toe (through the first interosseous space and under the metatarsal heads) to the neck of the fifth metatarsal.
