Summary
We describe the surgical technique of vitrectomy via the pars plana with our final instrumentation. The standardized procedure is based mainly on three original technical principles: the stripper like oscillating cutting mechanism, the use of a pilot-tube attached to the scleral incision and a new contact lens. The vitreous stripper is capable to cut easily all kind of vitreous structures as well as lens material and iris tissue. The pilot-tube facilitates the introduction of instruments into the vitreous cavity and the attachment of an infusion tube which maintains a normal intraocular pressure before and inbetween different steps of the surgical procedure. Holding the contact lens which is attached to the eye by sutures the surgeon may turn the eye in any desired direction. The technique has proved to be efficient in 25 patients.
Zusammenfassung
Nachdem wir das Instrumentarium fertig entwickelt haben, beschreiben wir unsere bereits weitgehend standardisierte chirurgische Technik der hinteren, über eine Öffnung in der pars plana ciliaris durchgeführten Vitrektomie. Als Neuentwicklungen haben sich besonders bewährt: der auf dem Stripper-Prinzip beruhende Vibrations-Schneidmechanismus, die Verwendung eines in der Sklera eingebauten Führungsröhrchens und ein neues Kontaktglas. Mit dem Vitreous-Stripper lassen sich alle Strukturen des pathologisch veränderten Glaskörpers, aber auch Linsenmaterial und Irisgewebe ohne Schwierigkeiten schneiden. Der Einbau eines Führungsröhrchens erleichtert das Einführen der chirurgischen Instrumente in den Glaskörperraum. Zur Aufrechterhaltung eines normalen intraokularen Druckes kann vor der Operation und zwischen den einzelnen Operationsphasen am Führungsröhrchen eine Infusion angebracht werden. Mit dem am Auge festgenähten Kontaktglas wird der Bulbus während der Operation in verschiedene Richtungen gedreht. Wir haben mit dieser Technik bereits 25 Patienten operiert.
