Destruktive Veränderungen an Wirbelkörpern bei der Spondylarthritis ankylopoetica

Zusammenfassung

Unter 371 Patienten mit einer Spondylarthritis ankylopoetica trafen wir in 3% der Fälle auf Zerstörungen des Bandscheibenraumes (Spondylodiscitis). An weiteren destruktiven Veränderungen fanden wir Zerstörungsvorgänge an der Wirbelkörpervorderkante, osteoporotische Wirbelkörpereinbrüche und intrasponginöse Hernien. An Hand von 11 Patienten mit Spondylodiscitiden wurde ihre Pathogenese erörtert, für die einmal eine Störung der metaplastischen Differenzierungsprozesse, zum anderen eine pannös-ulceröse Zerstörung des Gewebes oder ein lokalisierter osteoporotischer Knochenschwund mit sekundären Degenerationszeichen angenommen werden kann. Als ätiologische Faktoren sind in der Literatur die Cortisonbehandlung, Röntgenbestrahlung und traumatische Einflüsse diskutiert worden. Bei dem röntgenologisch gut umrissenen Bild einer Spondylodiscitis hebt sie sich gut gegen spezifisch, unspezifisch und rheumatisch spondylitische Prozesse ab. Die therapeutischen Möglichkeiten sind begrenzt und erschöpfen sich in medikamentösen Maßnahmen; in fortgeschrittenen Fällen kann die Spondylodese in Erwägung gezogen werden.

Summary

The authors report on their observations in 6% of 371 ankylosing spondylitis with destructive spondylitis. They present 11 patients with spondylodiscitis. They point out the polymorphism of this disease and discuss its interpretation. The clinical and radiological manifestations are well delimitated. The possibilities of the therapie are not yet sufficiently proved.

Résumé

L'auteur étudie 371 cas de maladie de Bechterew; 6% des cas montrent des destructions sur les corps vertébraux, dont la moitié des modifications dans la sphère des disques. Sur 11 malades présentant de tels processus disco-vertébraux la pathogénèse est discutée, pour laquelle on peut supposer une perturbation de la différenciation métaplastique, une destruction panno-ulcéreuse ou des phénomènes de dégénération. Du point de vue radiologique et clinique, la maladie se présente nettement délimitée. Les possibilités therapeutiques sont limitées.
