Traumatischer Muskelwulst oder Muskelhernie der unteren Extremität im Kindesalter

Summary

On the basis of 4 similar cases, the picture of the traumatic muscular protruberance of the lower extremity is brought to light and introduced. The picture is characterized by a nut- to tangerine-sized protruberance, usually in an exposed muscle of the lower extremity, and in accordance with the traumatic genesis, also painful during the initial period. An increase should not be surprising, and may give the impression of a growing tumour. However, this swelling, accompanied by pressure and walking difficulties, is a case of an overstretching of the fascia, which then gives in to the inside pressure of the muscle, or tonus. The treatment in each case should consist of an exposure, and, if a pursing of the fascia or a reduction of the muscle seems promising and likely to ease the difficulties, a biopsy.

Zusammenfassung

An Hand von 4 ähnlichen Fällen wird das Bild der traumatischen Muskelwulstbildung der unteren Extremität vorgestellt und damit zur Kenntnis gebracht. Das Bild ist gekennzeichnet durch eine nuß- bis mandarinengroße Wulstbildung in einem meist exponierten Muskel der unteren Extremität und entsprechend der traumatischen Genese in der Anfangszeit auch schmerzhaft. Eine Größenzunahme ist ohne weiteres glaubhaft, wodurch der Eindruck einer wachsenden Geschwulst hervorgerufen wird. Es handelt sich jedoch bei dieser mit Druck und Gehbeschwerden einhergehenden Wulstung um eine Zerdehnung der Fascie, welche dann dem Muskelinnendruck, dem Tonus, nachgibt. Die Behandlung besteht wohl jeweils in einer Freilegung und letztlich einer Probeexcision, falls man sich von einer Fascienraffung oder einer Muskelverkleinerung etwas verspricht, die dann die Beschwerdefreiheit mit sich bringt.
