Summary
A report is given upon electronmicroscopic observations in posttraumatic arthrosis. — Morphological criteria are desorganization of cytoplasmic membrane systems within the synovial lining cells. Filamentar elements are found within the relatively wide intercellular spaces in the vicinity of the joint cavity. — Thus the cells as well as the matrix differ markedly from those of normal ultrastructure. — Synovial cells in close neighborhood to capillaries appear relatively unaltered morphologically. The capillaries themselves display no alterations at least from the morphological point of view. — The importance of secretory and resorptive functions of the stratum synoviale is stressed. It is discussed in how far changes seen electronmicroscopically can influence the organ joint. In posttraumatic coxarthrosis those cells lying in a distance from the capillaries seem to be altered metabolically and therefore altered in their function, essential for the whole joint. This causes a structural change of the “Transitstrecke” which is assumed to play a role in sustaining the disease. — It is pointed out, that an altered secretion of the synovial cells results in dystrophic changes within the joint cartilage. Some prospects for therapeutic procedures are mentioned in connection with the results of this study.

L'auteur communique des observations sous microscope éléctronique portant sur le stratum synoviale en cas d'arthrose posttraumatique. — La caractéristique des cellules synoviales y est une désorganisation des systèmes membraneux zytoplasmatiques. Les espaces intercellulaires relativement grandes montrent des éléments filamenteux à proximité des cavités articulaires. Les cellules synoviales et, de même, la matrice intercellulaire se distinguent ainsi essentiellement de l'ultrastructure connue et normale. — Par contre, les cellules synoviales près des capillaires montrent une morphologie plus ou moins intacte. Les capillaires, eux-aussi, n'accusent aucune altération — au moins selon des critères morphologiques. — L'auteur souligne l'importance sécrétoire et résorbante du stratum synoviale et discute la question à savoir dans quelle mesure les altérations constatées sous microscope éléctronique influencent l'articulation dans son ensemble. — En cas d'arthrose post-traumatique, les cellules synoviales à distance des capillaires semblent être modifiées dans leur métabolisme et ainsi dans leurs fonctions essentielles pour l'articulation, entraînant un changement structurel du «parcours en transit» — ce qui pourrait être considéré comme facteur conservateur de la maladie. — L'attention est attirée sur les altérations dystrophiques du cartilage articulaire à la suite d'une sécrétion troublée des cellules synoviales. — Mention est faite des répercussions éventuelles que ces résultats pourraient avoir sur la thérapeutique.
Zusammenfassung
Es wurden elektronenoptische Untersuchungen am Stratum synoviale bei der posttraumatischen Arthrose durchgeführt. — Ein charakteristisches, morphologisches Kennzeichen der gelenklumennahen Synovialiszellen ist dabei eine Desorganisation der cytoplasmatischen Membransysteme. In den relativ weiten intercellulären Räumen finden sich gelenklumennah filamentäre Elemente. Die Synovialiszellen und ebenfalls die intercelluläre Matrix unterscheiden sich damit wesentlich von der bekannten Ultrastruktur im Normalfall. — Die Synovialiszellen in Capillarnähe erscheinen dagegen in ihrem morphologischen Bild relativ intakt. Auch die Capillaren weisen zumindest nach morphologischen Kriterien keine Veränderungen auf. — Die Bedeutung der sekretorischen und resorptiven Leistungen des Stratum synoviale werden hervorgehoben. Inwieweit sich die elektronenoptischen Veränderungen bei der posttraumatischen Arthrose auf das Gelenk in seiner Gesamtheit auswirken, wird diskutiert. — Bei der posttraumatischen Arthrose scheinen die capillarfernen Synovialiszellen in ihrem Stoffwechsel und damit in ihren für das gesamte Gelenk wesentlichen Funktionen verändert zu sein. Dabei tritt ein struktureller Wandel der Transitstrecke auf, der als ein krankheitserhaltender Faktor angesehen werden dürfte. — Es wird betont, daß bei einer gestörten Sekretion der Synovialiszellen dystrophische Veränderungen im Gelenkknorpel resultieren. — Die mögliche Bedeutung der Untersuchungsergebnisse für das therapeutische Vorgehen wird angedeutet.
