Gelenkinfektionen nach intraartikulärer Corticosteroidtherapie

Summary

The intraarticular application of corticosteroids in cases of non-bacterial arthritis has proven useful, and has reached considerable importance on orthopaedics. According of the increasing use of this method more reports appear about secondary infection of the joint involved. Several pharmacodynamic factors of glucocorticoids have an unfavorable influence upon the cours of these infections. Reporting on nine cases the means of prevention and eatly diagnosis of possible complications are discussed.

La corticothérapie intraarticulaire s'est montrée efficace dans les inflammations articulaires aseptiques de causes diverses, et on ne peut plus l'ignorer dans le pratique orthopédique. Mais le développement de cette méthode s'accompagne d'une augmentation des complications infectieuses articulaires. Différents facteurs qui dépendent de l'action propre des glycocorticoides influencent défavorablement le cours de ces infections. A partir de neuf observations personnelles les auteurs discutent des mesures permettant d'éviter ou de reconnaitre précocement d'éventuelles complications.

Zusammenfassung

Die intraartikuläre Corticosteroidtherapie hat sich bei nichtbakteriellen Gelenkentzündungen unterschiedlicher Genese bewährt und ist aus der ärztlichen Praxis nicht mehr wegzudenken. Mit zunehmender Verbreitung dieser Methode mehren sich die Berichte über komplizierende Gelenkinfektionen. Verschiedene Faktoren, die in der Wirkungsweise der Glucocorticoide begründet liegen, beeinflussen den Verlauf dieser Infekte ungünstig. An Hand von neun eigenen Beobachtungen werden Maßnahmen erörtert, die der Vermeidung bzw. frühzeitigen Erkennung möglicher Komplikationen dienen können.
