Summary
31 fractures have been observed upon 120 children with spina bifida cystica. The poor clinical picture with painless swelling, and local warming is frequently responsible for late diagnosis. Commonly trauma is inadequate and breaks a demineralized bone. The predisposition of juxta-articular portions is due to a lever mechanism. Fractures after plaster fixation for hip-operations are not uncommon. Rapid bony consolidation is generally assisted by luxurious callus. New fractures are prevented by early standing and walking in calipers. Fractures in spina bifida are compared with those in poliomyelitis, cerebral palsy and traumatic paraplegia.
Résumé
Nous avons observé 31 fractures du membre inférieur chez 120 enfants porteurs de spina bifida cystica. Le diagnostic clinique est souvent tardivement posé par suite de l'analgésie de ces patients; les signes cliniques sont le gonflement indolore et l'augmentation de la température locale. Il s'agit en général d'un traumatisme bénign d'un os déminéralisé. La prédominance des fractures juxta-articulaires s'explique par la longueur du bras de levier à ce niveau. La fixation platrée suivie de rééducation à la suite d'une intervention au niveau de la hanche est une cause fréquente de fractures. La consolidation est en général rapide, avec cal hypertrophique. L'appareillage avec mise en charge précoce est la meilleur prévention des fractures itératives. Les fractures pathologiques chez les spina bifida ont des caractérstiques différentes de celles des patients atteints de poliomyélite d'IMC ou de paraplégie posttraumatique.
Zusammenfassung
Bei 120 Kindern mit Spina bifida cystica wurden 31 Beinfrakturen beobachtet. Die dürftigen klinischen Zeichen mit schmerzloser Schwellung und lokaler Überwärmung führen häufig zu Spätdiagnosen. Das Trauma ist in der Regel nicht frakturadäquat und trifft demineralisierte Knochen. Die Bevorzugung der gelenknahen Regionen wird auf die Hebelwirkung an langen Röhrenknochen zurückgeführt. Die Gipsfixation nach Hüftoperationen prädisponiert zu Frakturen bei der nachfolgenden Übungsbehandlung. Die mit überschießender Callusbildung einhergehende Konsolidierung erfolgt ungewöhnlich schnell. Eine frühzeitige Apparatversorgung und Belastung schützt vor weiteren Brüchen. Die Besonder-heiten der Frakturheilung bei Spina-bifida-Kindern werden gegenüber den Befunden bei Poliomyelitis anterior, bei infantilen Cerebralparese und traumatischer Paraplegie abgegrenzt.
