Der Wert der Pfannendachplastik

The usefulness of acetabuloplasty of the hip

Summary

91 hips with congenital subluxation which were operated between 1947 and 1965 were followed up after the method of Spitzy and Lance. We examined the improvement of the a acetabular roof angles and the centering of head of the femur. We found the best results if the child was operated until the age of 3 years and if the acetabula plasty was done in combination with the osteotomy to correct anteversion and coxa valga. The so called hypertrophic acetabula rim had better results than the atrophic acetabula rim. The development of the acetabular roof was better if the hips were operated after the method of Lance. The autogenous grafts had less resorption than the homogenous grafts. Indications, technic of the operation and the influence of the acetabular roof for the process of revalgisation of the hip were discussed.

Zusammenfassung

91 zwischen 1947 und 1965 nach dem Verfahren von Spitzy und Lance operierte Hüftgelenke wurden in ihrem Verlauf kontrolliert. Die Entwicklung wurde nach dem AC- und dem CE-Winkel beurteilt. Die besten Resultate finden sich, wenn die Pfannendachplastiken in Kombination mit Osteotomien am Schenkelhals bis zum vollendeten 3. Lebensjahr des Patienten vorgenommen werden. Das Verfahren nach Lance ist dabei höher zu bewerten. Im Gegensatz zu dem sogenannten atrophen Pfannendach gibt das sogenannte hypertrophe Pfannendach bessere Ergebnisse. Eine wesentliche Bedeutung kommt der Zentrierung des Hüftkopfes zu. Der homologe Span (nach dem Verfahren von Spitzy) zeigt doppelt so häufig vollständige Resorptionen wie der autologe. Die Indikation, die Operationstechnik und der Einfluß des Pfannendaches auf die Revalgisierung werden diskutiert.
