Summary
Examination of 27 ruptures of the lateral ligament of the ankle treated by immobilisation and of 61 treated by surgical repair shows that only primary suture ensures mechanical stability. Conservative treatment leads to a defective healing of the ligament, followed by increasing instability, early arthrosis and corresponding complaints. Difficulties after operative treatment are dependent just on the process of healing of the ruptured ligaments. Possibilities of their overcoming are discussed.
Zusammenfassung
Die Verlaufsbeobachtung von 27 konservativ und 61 durch Bandnaht einschließlich Bandersatz versorgten Außenbandrupturen des Sprunggelenkes zeigt, daß allein die primäre Bandnaht Gelenkstabilität garantiert. Konservative Maßnahmen führen zu einer Bandheilung mit Defekt, die zunehmende Aufklappbarkeit, vorzeitigen Gelenkverschleiß und entsprechende Beschwerden nach sich zieht. Schwierigkeiten in der Nachbehandlung operativ versorgter Bandrupturen sind durch den Bandheilungsprozeß bedingt. Möglichkeiten ihrer Überwindung werden diskutiert.
