Summary
A 47-year-old man presented the compression signs of lumbar nerve roots L5 and S1. Beginning of the disease for 13 years, but low back pain was not intensive sine the last 6 months, when compression of the nerve roots became distinct and progressive. X-ray picture was not significant. The patient was operated upon. The large laminectomy demonstrated a tuberculous abscess coming from the interspace L5-S1. The nerve roots L5 and S1 were displaced and compressed. After excision of abscess and specific focus of S1 corps, the postoperative course was complicated by cold abscesses and fistulae typical for tuberculous process of vertebral column. After 2 years of adequate therapy, the patient is well and without complaints.
Zusammenfassung
Ein 47 jähriger Arbeiter präsentierte die Kompressionserscheinungen der Nervenwurzeln L5 und S1 links. Am Anfang der Krankheit vor 13 Jahren als mildere Kreuzschmerzen, die keine Arbeitsfähigkeitsbegrenzung verursachten. Vor 6 Monaten haben die Schmerzen sehr zugenommen, und seit 3 Monaten haben sich die Symptome einer Kompression der Nervenwurzel L5 und S1 eingestellt. Das Röntgenbild war nicht charakteristisch, außer Veränderungen der Knochenstruktur der untersten Lendenwirbel waren auch ausgiebige Osteophyten vorhanden. Die breite und adäquate Laminektomie hat einen tuberkulosen Absceß entdeckt, aus dem L5-S1-Zwischenwirbelraum ausgehend. Die Nervenwurzeln L5 und S1 waren verdrängt und komprimiert. Nach der Entfernung des Abscesses und Ausräumung des tuberkulösen Herdes des S1-Wirbels war der anfänglich glatte postoperative Verlauf durch Abscesse und Fistelbildung diesmal als typisch für Wirbelsäulentuberkulose kompliziert. Nach 2 Jahren adäquater Behandlung blieb der Patient beschwerdefrei und arbeitsfähig im begrenzten Ausmaß.
