Seltene Verletzungsfolgen an der Spina scapulae

Summary

The authors have put together the material belonging to the clinic and the literature published until now about the rare consequences of the injuries of the scapula (Pseudarthrosis of the scapula and of the acromion). In 3 cases of pseudarthrosis of the scapula we could observe functional damages. On the other hand the injuries of the acromion were all without any consequences. The differenciation of this injury towards the separate acromion is important and can eventually produce difficulties when an arthrosis deformans appears; in this case the traumatisation of an arthrosis latent until this time on the clinical basis can lead to the false diagnosis of a fracture, especially when it is due to arthrotic transformations the border of the acromial joint cannot be clearly identified and through the presence of the separate acromion often seen on one side, the comparison radiograph on the opposite side cannot always give information.

Résumé

Nous avons réuni le matériel appartenant á la clinique et la littérature publiée jusqu'à ce jour sur les suites rares des blessures de l'épine de l'omoplate (Pseudarthroses de l'épine et l'acromion). Dans 3 cas de pseudarthrose de l'épine, nous avons pu constater des pertes fonctionelles. Par contre les blessures de l'acromion étaient toutes sans suite. La différenciation de cette blessure envers l'os acromial est importante et peut éventuellement provoquer des difficultés lorsqu'il se présente une arthrose déformante de l'articulation intra-acromiale, car dans ce cas la traumatisation d'une arthrose jusqu'alors latente du point de vue clinique peut conduire au faux diagnostic d'une fracture, d'autant plus que par suite des transformations arthrotiques, on ne peut plus reconnaitre clairement le contour de l'articulation intra-acromiale et par la présence de l'os acromial souvent observé d'un coté, la radio de comparaison du coté opposé ne peut pas toujours donner d'éclaircissement.

Zusammenfassung

Das klinikeigene Material und die bisher veröffentlichte Literatur über seltene Verletzungsfolgen an der Spina scapulae (Pseudarthrosen der Spina und des Acromion) werden zusammengestellt. In 3 Fällen von Spinapseudarthrosen waren funktionelle Einbußen zu sehen. Die Acromionverletzungen waren sämtlich folgenlos. Die Abgrenzung gegenüber einem Os acromiale ist wichtig und kann evtl. bei Vorliegen einer Arthrosis deformans des Intraacromialgelenkes Schwierigkeiten machen, da hier die Traumatisierung einer bisher klinisch stummen Arthrose zur Fehldiagnose einer Fraktur führen kann, zumal bei der Arthrose infolge Randzackenbildung und subchondraler Sklerosierung die typische glatte Begrenzung des intraacromialen Spaltes nicht mehr deutlich erkennbar ist und bei dem häufigen einseitigen Vorkommen eines Os acromiale die Vergleichsaufnahmen der Gegenseite nicht immer Aufschluß geben.
