Summary
The modified McMurray's displacement osteotomy is regarded as indicated for mild coxarthrosis with congruent joint structures and satisfactory mobility in each age and for moderately advanced coxarthrosis below 60 years of age. Later on a total prosthetic joint replacement may be added, if necessary, so far as the medial femoral shaft displacement does not exceed 1.5 cms. The shortening of 1 cm as a result of simultaneous ascent of the plane of the osteotomy produces a marked muscle pressure releasing effect. Details of the operative procedure and the osteotomy plate are discussed.

L'ostéotomie de McMurray modifiée est recommandée aujourd'hui encore a tout âge dans les cas de légères coxarthroses où la congruence des articulations est bonne et où la mobilité est satisfaisante et dans les cas de coxarthroses modérement avancés avant 60 ans. Un peu plus tard, si cela est nécessaire, on peut procéder a une endoprothèse totale dans la mesure où la diaphyse du femur ne dépasse pas 1 cm,5. Le raccourcissement d'1 cm comme résultat d'une montée simultanée du niveau de l'ostéotomie produit un effet notable de détente du muscle. L'auteur discute des détails du procédé opératoire et de la plaque utilisée pour l'ostéosynthèse.
Zusammenfassung
Die modifizierte McMurraysche Verschiebeosteotomie ist heute noch bei leichten Coxarthrosen mit guter Gelenkkongruenz und ausreichender Beweglichkeit in jedem Alter und bei mäßig fortgeschrittenen Coxarthrosen im Alter unter 60 Jahren indiziert. Erforderlichenfalls kann später eine Totalendoprothese implantiert werden, sofern der Femurschaft um nicht mehr als 1,5 cm unterstellt wird. Die bei gleichzeitigem Anstieg der Osteotomieebene um 30° nach medial erzielbare Verkürzung von rund 1 cm ergibt einen starken muskelentspannenden Effekt. Besonderheiten des operativen Vorgehens und der zur Osteosynthese verwendeten Platte werden besprochen.
