Summary
One of the best methods for determine the AT- and CCD-angle is the method of Rippstein (1955). Precision and sources of mistakes of this method were examined with a femur in situ post mortem. The results were compared after the section with the anatomical preparation.
We could show the following: 1. The precision of the Rippstein-method ist also sufficient for scientific purposes if the positioning is done consciously. 2. While positioning, too strong a flexion (exceeding 90°) can causes more often a mistake than a flexion which is too little. 3. Too feeble an abduction (less than 20°) leads more often to mistakes than too strong an abduction. 4. Exterior rotation causes an increase of the projected AT-angle, while interior rotation leads to a decrease of that angle. These results mentioned under 1 to 4 were represented in a system of coordinate lines. — In the same way, the X-ray-projection or wrong projection of the collum femoris angle on the a.p.-radiograph were registered for various positions of flexion and rotation of the femur.
Résumé
La précision ainsi que les sources d'erreurs possibles de la méthode radiologique pour la détermination de l'angle antetorsion et du col du fémur (angles AT et CCD) selon Rippstein (1955) furent définies avec un fémur post mortem se trouvant in situ et les résultats obtenus furent comparés après la dissection avec la préparation anatomique.
Le suivant en résulta: 1. La précision de la méthode Rippstein est suffisante aussi pour les recherches scientifiques si la position est choisie consciencieusement. 2. En choisissant la position juste, une flexion trop forte (dépassant 90 dégrés) est une source d'erreurs plus importante qu'une flexion trop faible. 3. Une abduction trop faible (inférieure à 20°) est une source d'erreurs plus importante qu'une abduction trop forte. 4. La rotation à l'extérieur amène une augmentation de l'angle AT projeté, la rotation à l'intérieur amène une diminuition de cet angle. Les résultats cités sous 1 à 4 furent représentés dans un système de coordonnées. — De la même façon, la projection radiologique c. à. d. la projection erronnée de l'angle du col du fémur sur la radiographie a.p. fut régistrée en choisissant plusieurs positions de flexion et de rotation de la cuisse.
Zusammenfassung
Genauigkeit und Fehlerquellen der röntgenologischen Methode zur Bestimmung des Antetorsions- und Schenkelhalswinkels (AT- und CCD-Winkels) nach Rippstein (1955) wurden an einem in situ befindlichen Femur post mortem bestimmt und die gefundenen Werte nach der Sektion mit dem anatomischen Präparat verglichen.
Es ergab sich folgendes: 1. Die Genauigkeit der Rippsteinschen Methode reicht bei sorgfältiger Lagerung auch für wissenschaftliche Zwecke aus. 2. Bei der Lagerung ist eine zu starke Beugung (mehr als 90°) eine größere Fehlerquelle als eine zu geringe Beugung. 3. Eine zu geringe Abduktion (weniger als 20°) ist eine größere Fehlerquelle als eine zu starke Abduktion. 4. Außenrotation führt zu einer Verstärkung und Innenrotation zu einer Verminderung des projizierten AT-Winkels. — Die unter 1–4 aufgeführten Ergebnisse wurden im Koordinatensystem zeichnerisch dargestellt. In gleicher Weise wurde die röntgenologische Projektion bzw. Fehlprojektion des Schenkelhalswinkels auf dem a.p.-Bild bei verschiedener Beuge- und Rotationsstellung des Oberschenkels aufgezeichnet.
