Summary
By running 40 fresh human at least 65 years old femoral heads through a tensil test machine it was found out that the spongious part of the femoral head tolerates traction up to 330 kg in the highest and 150 kg in the lowest case.
Since axial pressure of aproximately 80 to 90 kg is sufficient to provide training stability in a femoral neck fracture, there is a safety-factor of 87.5% in the most disadvantageous case.
Measurings by a pressure stamp in 0.5 cm thick femoral head disks show, that the compact nucleous of the femoral head tolerates pressure up to 230 kg/cm2, while only 7 to 24 kg/cm2 are tolerated in most of the peripheric parts.
It is therefore concluded, that the screw we use to stabilize a femoral neck fracture by axial tension is safely fixed in the femoral head when inserted centrally.
Zusammenfassung
Durch Versuche an 40 postmortal entnommenen Hüftköpfen wurde an einer geeichten Zerreißmaschine ermittelt, daß Hüftkopfspongiosa Zugbelastungen bis zu 330 kg im Maximalfall und 150 kg im Minimalfall toleriert.
Da für eine Übungsstabilität nach Kompressionsverschraubung einer Schenkelhalsfraktur eine axiale Spannkraft von 80 bis 90 kg ausreicht, resultiert für die mechanische Zugbelastbarkeit der Kopfspongiosa ein Sicherheitsbereich von 87,5% im ungünstigsten Fall.
Messungen mit einer Drucksonde an 0,5 cm dicken Hüftkopfscheiben ergaben, daß der kompakte Hüftkopfkern mit bis zu 230 kg/cm2 belastet werden kann, gegenüber 7 bis 24 kg/cm2 in den meisten Randbezirken.
Hieraus kann abgeleitet werden, daß die von uns verwendete Schenkelhalsschraube, die die Frakturflächen unter axialem Druck stabilisieren soll, bei zentraler Lage im Schenkelhalskopf eine ausreichend feste Verankerung findet.
