Zur operativen Behandlung der Osteochondrosis dissecans des Hüftgelenkes

Zusammenfassung

Wir haben an vier Hüftgelenken bei drei Patienten wegen einer vollausgebildeten Osteochondrosis dissecans die varisierende, intertrochantere Keilosteotomie mit zusätzlicher Herdanbohrung durchgeführt und konnten an allen vier Erkrankungsherden eine Ausheilung beobachten. Wir glauben daher, daß derzeit die Pauwelsche Adduktionsosteotomie bei der Osteochondrosis dissecans der Hüfte mit valgischem Schenkelhals die Methode der Wahl darstellt. Die den Eingriff in keiner Weise belastende Anbohrung des kranken Herdes wird zusätzlich empfohlen.

Summary

In four cases of fully developed osteochondrosis of the hip joint we have performed on three patients varus wedgeformed osteotomy with additional trepanation of the focus. In every one of the four cases we could effect complete reconstruction. Therefore we think that at present the osteotomy in form of adduction according to Pauwels is the best method of treatment of the osteochondrosis of the hip joint if a valgus of the femoral neck exists. In addition we recommend the trepanation of the focus since there is no additional risk worth mentioning.

Raport de quatre cas d'osteochondrose dissécante de la hanche complétement developée chez trois malades. Nous avons fait une ostéotomie intertrochanterienne cuneiforme de varisation avec trépanation du focus. En chaque cas nous avons obtenue une guérison complette. C'est pourquoi nous croyons que lóstéotomie d'adduction selon Pauwels est la méthode meilleure pour le traitement de l'osteochondrose dissécante de la hanche avec un col fémoral en valgus. En outre nous conseillions la trépanation du focus comme elle ne représente aucun risque additionel.
