Summary
An analysis of 55 reoperations on 36 patients after the implantation of complete alloarthroplasty is presented. The infections with 27 operations make up the largest group. These are devided into early (15), lingering (3), and late infections (9). The remainder of the first reoperations were necessary because of prosthesis, loosening without infection (4), traumatic diaphysis fracture (2), recurrent luxation (1), failure to diagnose a diaphysis perforation and impaired healing of the wound (1), in 21 cases the whole prosthesis was removed, in 7 cases both parts and in 2 cases the socket was replaced. Of the 12 total or partial prosthesis replacements 5 had to be removed in further operations. From the relatively long interval between implantation and removal (3–50 months) (implantation-replacement 4–20 months) it may be deduced that manifest infections should be examined early to retain the prosthesis or at least to increase its chances of replacement.
Inquiry into the case history is important to diagnose the nature of the pain correctly, as are clinical, serologic and radiological examinations for early recognition of an infection of loosening.
The bacteria cultures grown from the wound smear are analysed and here we find that in early infections gram-negative germs were slightly more frequent than gram-positive ones, and in late infections, infections from both groups are equally frequent.
Particular attention is drawn to the fact that very stringent indication is necessary with systemic disorders such as diabetes mellitus or Hodgkin's disease.
Zusammenfassung
Es wird eine Analyse von 55 Reoperationen bei 36 Patienten nach der Implantation von Totalalloarthroplastiken vorgelegt. Das Hauptkontingent mit 27 Operierten stellen die Infektionen dar, die in Früh- (15), schleichende (3) und Spätinfekte (9) eingeteilt werden. Die übrigen ersten Reoperationen wurden erforderlich wegen Prothesenlockerungen ohne Infektionen (4), traumatischen Schaftfrakturen (2), rezidivierende Luxation (1), nichterkannte Schaftperforation (1) und sterile Wundheilungsstörung (1). 21mal wurde die Total-prothese entfernt, 7mal beide Teile, 3mal der Schaft und 2mal die Pfanne gewechselt. Von diesen 12 gewechselten Total prothesen bzw. Prothesenteilen mußten schließlich 5 in einer weiteren Operation entfernt werden. Aus dem vergleichsweise langen Intervall zwischen Implantation und Entfernung (3–50 Monate) (Implantation—Auswechslung 4–20 Monate) wird die Forderung abgeleitet, bei manifesten Infektionen frühzeitig zu revidieren, um die Prothese zu erhalten bzw. die Chance ihrer Auswechslung zu erhöhen.
Wichtig sind die anamnestische Befragung, um den Schmerzcharakter richtig zu deuten, sowie die klinische, serologische und röntgenologische Untersuchung, um Infektionen oder Lockerungen frühzeitig zu erfassen.
Die aus den Wundabstrichen gezüchteten Bakterienkulturen werden analysiert, wobei sich zeigt, daß bei den Frühinfektionen die gramnegativen Keime etwas häufiger vertreten sind als die grampositiven und bei den Spätinfektionen beide Gruppen etwa gleich häufig vorkommen.
Hingewiesen wird auf die Notwendigkeit der besonders strengen Indikationsstellung bei heilungsbeeinträchtigenden Allgemeinkrankheiten, wie Diabetes mellitus oder Morbus Hodgkin.
