Knöcherne Auffüllung des Distraktionsspaltes bei Beinverlängerung

Osseous bridging of the distraction gap in leg lengthening procedures

Summary

The bony refill of the distraction-space in leg-lengthening operations is analysed by X-ray control and histological technic as well in animal experiments as clinical patients. Based on our observations a theory of the formation of bone is postulated:

We figured out, that

  1. a)

    presumable besides the periosteal potentials of bone formation there are also other active mechanisms of bone growth to be found.

  2. b)

    frequently histological recognised formations of bones cannot yet be proved by X-ray, that means, that the X-ray changes follow the histological ones.

  3. c)

    optimal formation of callus needs most possible stability during the time of consolidation.

  4. d)

    most careful distraction prevents deformities.

Zusammenfassung

Bei Beinverlängerungen wird die knöcherne Auffüllung des Distraktionsspaltes röntgenologisch und histologisch sowohl tierexperimentell als auch klinisch untersucht. Aufgrund unserer Beobachtungen wird ein theoretischer Ablauf der Knochenbildungsvorgänge postuliert.

Es zeigte sich, daß

  1. a)

    vermutlich neben der periostalen Knochenbildung noch andere Verknöcherungsmechanismen anzutreffen sind.

  2. b)

    histologisch erkennbare Knochenbildungen röntgenologisch oft noch nicht nachweisbar sind, d. h., daß das Röntgenbild hinter den histologischen Veränderungen nachhinkt.

  3. c)

    interfragmentär optimale Callusbildung während der Konsolidierungsphase größtmöglicher Stabilität bedarf.

  4. d)

    möglichst subtile Distraktion Fehlstellungen vermeiden kann.
