Über die Cerclage bei Unterschenkelbrüchen

Zusammenfassung

Die Erfolge und Erfahrungen mit dem Verfahren der Drahtcerclage bei Brüchen des Unterschenkels werden an 194 Fällen der II. Chirurgischen Klinik erläutert. Folgende Prinzipien werden herausgestellt:

  1. 1.

    Die Drahtcerclage ist keine Fixation, sondern dient zur Retention von Fragmenten; es ist die Extremität daher unbedingt durch einen langen bis auf den Oberschenkel reichenden Gipsverband zusätzlich ruhig zu stellen.

  2. 2.

    Nur genügend lange Schrägfrakturen erscheinen für eine Cerclage geeignet (ab 4,5 cm zwei Drahtschlingen, 4,5 bis 3 cm eine Drahtschlinge), Stückbrüche nur, wenn sie offen sind und wenn sich die beiden Hauptfragmente noch überschneiden. Splitterbrüche sind ungeeignet! Bei Kindern wird nicht cercliert, bei Jugendlichen nur ausnahmsweise.

  3. 3.

    Die Drahtschlingen müssen mindestens anderthalb Zentimeter voneinander und von den Frakturspitzen entfernt angelegt werden. In der Regel ist die Verwendung von mehr als zwei Drähten unangebracht.
