Zusammenfassung
1. Brüche des radialen Oberarmcondyls bei Kindern ohne Verschiebung werden mit einem Oberarmgipsverband für 3 Wochen ruhiggestellt. Die Nachuntersuchung zeigt gute Ergebnisse.
2. Brüche des radialen Oberarmcondyla bei Kindern mit geringer Verschiebung werden konservativ eingerichtet. Gelingt die Reposition, werden sie für 4 Wochen mit einem Oberarmgipsverband ruhiggestellt. Gelingt die Reposition nicht oder kommt es bei der Röntgenkontrolle nach einer Woche zu einer neuerlichen Verschiebung, sollen sie operiert werden.
3. Brüche des radialen Oberarmcondyls mit starker Verschiebung sollen operiert werden, da die unblutige Einrichtung fast nie gelingt. Die Fixation mit zwei Bohrdrähten hat sich gut bewährt. Sie werden anschließend mit Oberarmgipsverband für 4 Wochen ruhiggestellt. Die Bohrdrähte sollen entfernt werden, sobald die Haut nach Gipsabnahme in Ordnung ist.
4. Brüche des radialen Oberarmcondyls mit Verrenkung des Vorderarmes nach ulnar (Posadas-Typ) können konservativ eingerichtet werden und bleiben im Oberarmgips für 4 Wochen stabil.
Bei Brüchen des radialen Oberarmcondyls mit Verrenkung des Vorderarmes nach radial (Kocher-Typ) mußten wir blutig reponieren und mit Bohrdrähten fixieren.
Bei Beachtung dieser Punkte sind die Behandlungsergebnisse bei Brüchen des radialen oberarmcondyls im Kindesalter gut.
Summary
1. Fractures of the radial upper arm condyl without displacements in children are immobilised with an upper arm plaster dressing for 3 weeks. Re-examinations showed good results.
2. Fractures of the radial upper arm condyl with slight displacements in children are reduced in a conservative fashion. If the reduction is successful they will be immobilised with an upper arm plaster dressing for 4 weeks. If the reduction does not succeed or a fresh displacement takes place during the radiologic control a week later, the fractures should be operated.
3. Fractures of the radial upper arm condyl with strong displacements should be operated, because a bloodless setting almost never succeeds. The fixation with 2 perforating wires has proved a success. Afterwards the fractures are immobilised with an upper arm plaster dressing for 4 weeks. The wires are to be removed as soon as the skin after removal of the plaster is well again.
4. Fractures of the radial upper arm condyl with luxations of the forearm in the ulnar direction (Posadas type) can be readjusted in a conservative manner and remain stable in an upper arm plaster for 4 weeks.
Fractures of the radial upper arm condyl with luxations of the forearm in the radial direction (Kocher type) had to be reduced in a bloody way and fixed with above mentioned wires.
If one pays attention to these points treatment results in fractures of the radial upper arm condyl will be good during infancy.
Résumé
1. Les fractures du condyle radial de l'humérus sans désajustement chez les enfants sont immobilisées pendant trois semaines avec un moule du bras en plâtre. Les examens secondaires montrent de bons résultats.
2. Les fractures du condyle radial de l'humérus chez les enfants sont remboîtées d'une manière conservative si le désajustement est moindre. Si la réposition réussit, elles sont immobilisées pendant 4 semaines avec un moule en plâtre. Si la réposition ne réussit pas ou si aprés une semaine il s'est produit un nouveau désajustement pendant le contrôle radiographique, elles doivent être opérées.
3. Les fractures du condyle radial de l'humérus avec un fort désajustement doivent être opérées, car la remise en position normale non sanglante ne réussit presque jamais. La fixation avec deux fils perforateurs s'est bien confirmée. Elles sont ensuite immobilisées pendant 4 semaines dans un moule en plâtre. Les fils perforateurs doivent être enlevés dés que la peau se rétablit aprés l'enlèvement du plâtre.
4. Les fractures du condyle radial de l'humérus avec luxation de l'avant-bras d'un type ulnaire (type Posadas) peuvent être remboîtées d'une manière conservative et restent stables dans un plâtre pendant 4 semaines.
Pour les cas de fractures du condyle radial de l'humérus avec luxation de l'avant-bras d'un type radial (type Kocher) il faudrait réduire d'une manière sanglante et les fixer avec des fils perforateurs.
Si l'on considère ces points, les résultats du traitement en cas de fractures du condyle radial de l'humérus chez les enfants sont bons.
