Zusammenfassung
Die Ergebnisse der konservativen Behandlung von Schienbeinkopfbrüchen sind im großen und ganzen immer noch unbefriedigend. Unsere in der vorliegenden Arbeit mitgeteilten Nachuntersuchungen von 61 konservativ behandelten Schienbeinkopfbrüchen deuten darauf hin, daß sie entscheidend beeinflußt werden einerseits von der primären Fragmentverlagerung und dem Gelingen der Reposition und andererseits vom Lebensalter der Patienten. Dabei besteht die eigentliche Bedeutung der Dislokation bzw. Reposition darin, daß sie einmal bei entsprechendem Ausmaß bzw. ungenügender Beseitigung von sich aus die statischen und funktionellen Leistungen des Kniegelenks beeinträchtigen können, und daß sie zum anderen die maßgebliche Verantwortung für die Entstehung einer posttraumatischen Arthrosis deformans tragen. In welchem Umfange sich jedoch eine unvollkommene Brucheinrichtung bzw. eine posttraumatische Arthrosis deformans auf die Kniegelenksfunktionen bemerkbar macht, hängt weitgehend vom Alter des Verletzten ab. Vom 5. Lebensjahrzehnt ab überwiegen die unbefriedigenden Behandlungsergebnisse. Die Wahl des Behandlungsverfahrens bei Schienbeinkopfbrüchen verlangt daher auch stets eine genaue Beachtung des Alters des Verletzten, und zwar in der Weise, daß im Hinblick auf eine ideale Bruchkorrektur mit zunehmendem Patientenalter noch höhere Anforderungen gestellt werden müssen. Ist eine vollständige Reposition auf konservativem Wege nicht zu erreichen oder liegt eine Fraktur vor, die erfahrungsgemäß mit hoher Wahrscheinlichkeit bei konservativer Behandlung zu einem unbefriedigenden Ergebnis führt, so sollte, zumindest vom 40. Lebensjahr an aufwärts, die operative Brucheinrichtung vorgenommen werden.
Summary
On the whole, results in conservative treatment of tibia head fractures have still been unsatisfactory. Our re-examinations of 61 conservatively treated tibia head fractures, which are dealt with in the present paper, point out to the fact that, on the one hand, these fractures are decisively influenced by the primary displacement of the fragments and a successful reduction and, on the other hand, by the patients' age. In this connection the proper meaning of the dislocation resp. reduction lies in the fact that, on the one hand, in case of corresponding proportions resp. an unsatisfactory readjustment it can, by itself, handicap the static and functional performances of the knee joint and, on the other hand, can be made responsible for the formation of a post-traumatic arthrosis deformans. To what extent an imperfect fracture setting resp. a post-traumatic arthrosis deformans will make itself felt in the functioning of the knee joint, greatly depends on the patient's age. From the age of 40 onwards unsatisfactory results in treatment will prevail. That is why the choice of the treatment method for shinbone head fractures alway requires strict attention to the age of the injured, and that in such a way as to put up — with regard to an ideal fracture setting — higher standards for patients of increasing age. In case a complete reduction cannot be obtained through the conservative way or a fracture occur that, according to medical experience, will probably have no satisfactory result if submitted to a conservative treatment, then a surgical fracture setting should be carried out, at least from the age of 40 onwards.
Résumé
Les résultats du traitement conservatif de la fracture de tête du tibia sont restés jusqu'ici en somme insuffisants. Nos examens secondaires de 61 fractures de tête du tibia conservativement traitées, qui sont communiqués dans notre travail présent, signalent le fait que celles-ci sont influencées décısivement d'une part par le déplacement primaire des fragments et la réussite de la réposition et d'autre part par l'âge du patient. La propre signification de la dislocation resp. réposition consiste en ce que d'une part elle peut restreindre par elle-même, en présence des dimensions correspondantes resp. d'une élimination insuffisante, les travaux statiques et fonctionnels de l'articulation du genou et que d'autre part elle porte la responsabilité compétente pour la naissance d'une arthrosis deformans posttraumatique. Il dépend largement de l'âge du blessé, dans quelle étendue cependant se fait remarquer une réduction imparfaite de la fracture resp. une arthrosis deformans posttraumatique sur la fonction de l'articulation du genou. Entre 40 et 50 ans les résultats de traitement insatisfaisants prédominent. Le choix de la méthode de traitement en cas de fracture de la tête du tibia exige aussi constamment que l'on prenne en considération l'âge du blessé, et cela d'une façon, qu'en raison de l'âge croissant, des exigences plus grandes doivent être mises à l'égard d'une rectification idéale de la fracture. Si une réposition intégrale par la méthode conservative ne peut être atteinte ou si une fracture se présente, qui, suivant l'expérience, mène en cas de traitement conservatif avec une grande probabilité à un résultat insatisfaisant, l'ajustement opératif de la fracture devrait être entrepris au moins à partir de la quarantaine.
