Summary
A 72-year-old adult-onset diabetic woman underwent vitrectomy with the vitrophage because of persistent vitreous hemorrhage. Postoperatively a hyphema developed and elevation of intraocular pressure was refractory to medical management. Intraocular pressure was controllable within normal limits after vitreous lavage. Eleven days after vitreous lavage the patient died from pulmonary embolization. Histologic sections demonstrated a flat pars plana over the sclerotomy site with vitreous condensation along the internal aspect of the wound. Rubeosis iridis was postulated as the source of the hyphema. A hemolytic mechanism may have caused the elevated intraocular pressure.
Zusammenfassung
Bei einer 72jährigen, zuckerkranken Frau wurde eine Vitrektomie mit dem Vitrophagen wegen andauernder Glaskörperblutung durchgeführt. Nach der Operation zeigte sich eine Vorderkammerblutung und eine Erhöhung des intraokularen Druckes, die medikamentös nicht beeinflußbar war. Durch eine Glaskörperlavage konnte der Druck normalisiert werden. Elf Tage später starb die Patientin an einer Lungenembolie. Die histologische Untersuchung zeigte, daß der Strahlenkörper an der sklerotomierten Stelle nicht abgehoben war. Glaskörperverdichtungen fanden sich an der Innenseite der Wunde. Eine Rubeosis iridis wurde als Quelle der Vorderkammerblutung angenommen. Ein „hämolytisches Glaukom“ war wahrscheinlich der Grund der Druckerhöhung.
