Zusammenfassung
Die klinische Wirksamkeit psychtroper Arzneien ist schwer zu beurteilen. Massenstatistiken haben ihre Grenze an den individuellen Verlaufs — und Syndromvarianten. Statt dessen kann man mit dem individuellen Vorverlauf vergleichen. Am optimalen Vorverlaufsabschnitt wird der während einer Versuchsbehandlung erzielte Erfolg gemessen. Rechnerische Vergleichbarkeit schaffen graphische Darstellung und Planimetrie (der von den Verlaufskurven umschriebenen Felder). Größerer Erfolg als früher drückt sich anschaulich in entsprechenden Zahlenverhältnissen aus. Das Verfahren wird am Modell von neun chronischen oder schubweise rezidivierenden schizophrenen Verläufen exemplifiziert. Nach dem unmittelbaren klinischen Eindruck hatte sich die Symptomatik während einer Fluphenazin-Behandlung gut gebessert. In der Kontrolle ergibt sich aber, daß diese Besserung den optimalen früheren Besserungen nicht signifikant überlegen ist. Das ist noch kein negatives Urteil über die Versuchstherapie. Es besagt lediglich, daß eine Wirksamkeitsüberlegenheit nicht objektiviert ist und spontane Remissionstendenzen nicht ausgeschlossen sind.
Summary
The clinical efficiency of psychoactive drugs is difficult to criticize. Statistics are limited by the variants of the individual courses and symptoms. Instead of those the improvement under a new drug can be compared with the foregoing improvements of the same case, and that with the best foregoing improvement. The degree and time of the improvement are plotted by a curve bounding fields. These fields are measured by planimetry. The greater an improvement is, the bigger the field. The numerical value of the field representing the best previous improvement is divided by that representing the drug effect.
The method is exemplified by means of 9 chronic or relapsing schizophrenic courses. According to the immediate clinical impression the symptoms grew better during Fluphenazine treatment. However, as follows from the control this improvement does not surpass significantly the optimal foregoing improvements. Therefore spontaneous remission tendencies are not completely excluded; at least the superiority of Fluphenazine is not reliably proved.
