Summary
Following a review of the literature EEG findings are described obtained from 44 electroconvulsions (EC) of psychotic patients under general anaesthesia and muscular relaxation. In accordance with other authors 4 different stages are distinguished: latent, tonic, clonic and postparoxysmal. The postparoxysmal stage begins simultaneously in all records with isoelectricity. This is sometimes not fully pronounced over that hemisphere which showed, less developed spikes, during EC.
The EEG during EC is compared with the EEG during Metrazol convulsions. The main differences are the shorter duration of the seizure and the absence of the initial clonic stage in EC, and the relatively frequent recurrence of the paroxysm in MC, which was not observed in EC. The findings during EC support some conclusions of the preceding paper on MC. A caudato-thalamic antiepileptic inhibitory system influencing the course of the paroxysm is postulated. These therapeutic convulsions are considered to be a model of spontaneous major epileptic seizures and their study may be valuable for the neurophysiological interpretation of grand mal.
Zusammenfassung
Nach einer kurzen Literaturübersicht beschreiben die Autoren EEG-Befunde bei 44 Elektrokrämpfen (EK) in Narkose und Relaxation. Übereinstimmend mit anderen Autoren wurden vier Stadien beobachtet: 1. Latenz-, 2. tonisches, 3. klonisches, 4. postparoxysmales Stadium. Letzteres beginnt mit einer isoelektrischen Linie synchron über allen Ableitungsbereichen. Sie ist mit-unter etwas unvollkommener auf der Seite, die im Vergleich zur gegenüberliegenden während des-EK Spitzenpotentiale in etwas geringerer Ausprägung zeigte.
Das EEG des EK wird mit dem des Pentamethylentetrazolkrampfes (PK) verglichen. Die Hauptunterschiede betreffen die Gesamtlänge des Krampfanfalles (wesentlich längere Dauer des PK), das Fehlen des klonischen Vorstadiums bei EK, die Wiederholung oft sogar des ganzen Paroxysmus beim PK, was beim EK nicht der Fall war. EEG-Befunde beim EK unterstützen die Schlußfolgerungen, die die Autoren in der Arbeit über das EEG beim PK gezogen haben. Es wird ein caudatothalamisches antiepileptisches Inhibitionssystem angenommen, um den Verlauf der Anfälle sowie die Unterschiede zwischen PK und EK zu erklären. Ferner sind unterschiedliche Wirkungsmechanismen beider Methoden der Krampfauslösung von Bedeutung. Die beim Heilkrampf als idealem Modell gewonnenen Erkenntnisse werden auf die Interpretation des spontanen epileptischen Anfalls angewandt.
