Summary
This report deals with a 5 year follow up study on an initially 8 year old girl with polyostotic lesions.
She had several pathological fractures. All but one healed without internal fixation and without delay.
The non healing fracture located in a lesion at the distal part of the humerus had to be treated by extra periosteal excision of the osseous lesion and by bridging the gap with an homologous bone graft. An extensive lesion near the epiphyseal zone of the proximal femur was treated in the same manner.
After 15 months the metal plates were removed and bone biopsies taken of the femoral graft where a new lesion seemed to be radiologically apparent. The histological findings confirmed the suspicion of a new lesion of fibrous dysplasia.
4 years later a fracture occured in this region which healed without delay. The radiological findings now confirmed the presence of a new lesion in the grafted bone.
There is no general necessity to use internal fixation in pathological fractures of fibrous dysplasia.
However if internal fixation is necessary the excision of the diseased bone together with removal of the periosteum is obligatory to prevent a relapse in the same area. This case demonstrates however that even here a new lesion may occur in the remodelled bone graft.
Although this disease is primarily found in growing bone, a close follow up subsequent to adolescence is necessary to establish a possible onset of malignancy.
Zusammenfassung
Am Beispiel einer 5jährigen Ver laufsbeobachtung eines damals 8jährigen Mädchens mit polyostotisch fibröser Dysplasie werden die Behandlungsmöglichkeiten und die Problematik der Rezidivgefährdung nach Periostresektion und homologer Vollknochentransplantation erörtert.
Monostotische Herde sollten zur Sicherung der Diagnose ausgerdumt und mit autologer Spongiosa aufgefüllt werden.
Bei gesicherter Diagnose kann die Heilung pathologischer Frakturen auch auf konservativem Wege an gestrebt werden.
Größere Defekte sollten mit autologem oder homologem Knochenmaterial nach Resektion des Periostschlauches überbrückt werden, wobei dies vor einem Rezidiv jedoch nicht schützen kann.
Eine Röntgenbestrahlung der Herde ist wegen der möglichen malignen Entartung abzulehnen.
Die spontane Entartungsh:aufigkeit von 0,8% nach durchschnittlich 14 Jahren unterstreicht die Notwendigkeit einer sorgfältigen Kontrolle dieser Patienten über einen sehr langen Zeitraum ihres Lebens auch nach Abschluß der Skelettentwicklung.
